| 目的：研究血清视黄醇结合蛋白（RBP）、同型半胱氨酸（Hcy）、胱抑素C（Cys C）及尿微量白蛋白（mALB）水平在糖尿病肾病
早期诊断中的临床价值，为临床诊疗提供依据。方法：选取2014 年6 月到2016 年2 月我院收治的2 型糖尿病患者138 例，根据
24 小时尿蛋白排泄率将患者分为单纯糖尿病（A 组65 例），早期糖尿病肾病（B 组73 例），另选取同期健康体检者65 例为对照
组，检测各组血清中RBP、Hcy、Cys C及尿m ALB 水平。结果：B组RBP、Hcy、Cys C及尿m ALB 水平均显著高于A 组和对照
组，比较差异具有统计学意义（P＜0.05），A 组RBP、Hcy、Cys C显著高于对照组，比较差异具有统计学意义（P＜0.05）；RBP、Hcy、
Cys C及尿mALB联合检测阳性率显著高于单独检测（P＜0.05）。结论：早期糖尿病肾病患者血清RBP、Hcy、Cys C及尿mALB
水平显著上升，联合检测能提高检测阳性率。 |

| Objective:To study the clinical value of Serum retinol binding protein(RBP), homocysteine(Hcy), Cystatin C(Cys C)
and urinary albumin (M ALB)level in the early diagnosis of diabetic nephropathy, to provide basis for clinical diagnosis and treatment.Methods:138 cases with type 2 diabetes treated in our hospital fromJune 2014 to February 2016 were selected, the patients were divided
into 65 groups: diabetic nephropathy (group A),early diabetic nephropathy (group B, 73 cases) according to the 24 hour urinary protein
excretion rate, another 65 cases of healthy physical examination were selected as the control group, detection of serum levels of RBP,
Hcy, Cys C and urinem ALB in each group.Results:The levels of RBP, Hcy, Cys C and urine ALB in group B were significantly higher
than those in group A and control group, the difference was statistically significant (P<0.05), the levels of RBP, Hcy, Cys C in group A
were significantly higher than the control group, the difference was statistically significant(P<0.05), the positive rates of RBP, Hcy, Cys C
and urinem ALB were significantly higher than those in the single test (P<0.05).Conclusion:SerumHcy, RBP, C Cys and urine mALB
of early diabetic nephropathy patients will be elevated, combined detection can improve the positive rate of detection. |
