      摘要 目的:本研究旨在探讨不同FEV1的支气管扩张症患者气道炎症与肺功能损害的相关性及肺功能受损的影响因素分析。方法:选取2022年5月至2024年1月期间在本院呼吸内科收治的80例支气管扩张症患者作为研究对象,分析各组一般资料,对比各组肺功能、气道炎性细胞因子水平,采用Pearson直线分析法分析结果显示支气管扩张症患者肺功能与气道炎症的关系,采用多因素 Logisitic 回归分析支气管扩张症患者肺功能受损的影响因素。结果:将三组的FVC、FEV1/FVC、DLCO评估纳入研究并实施组间差异性比较,结果显示,高FEV1组、中FEV1组FVC、FEV1/FVC、FVC高于低FEV1组,高FEV1组FVC、FEV1/FVC、FVC高于中FEV1组(P<0.05)。将三组的ESR、CRP、WBC、NEUT、PCT、IL-4、IL-6、IL-10、TNF-α水平评估纳入研究并实施组间差异性比较,结果显示,高FEV1组、中FEV1组ESR、CRP、WBC、NEUT、PCT、IL-4、IL-6、TNF-α水平低于低FEV1组,IL-10水平高于低FEV1组(P<0.05);高FEV1组ESR、CRP、WBC、NEUT、PCT、IL-4、IL-6、TNF-α水平低于中FEV1组,IL-10水平高于中FEV1组(P<0.05)。采用Pearson直线分析法分析结果显示,支气管扩张症患者FEV1、FVC、FEV1/FVC、DLCO与气道炎症因子ESR、CRP、WBC、NEUT、PCT、IL-4、IL-6、TNF-α均呈正相关,与IL-10呈负相关(P<0.001)。自变量设置为上述资料中对比有差异的指标包括FVC、FEV1/FVC、DLCO、ESR、CRP、WBC、NEUT、PCT、IL-4、IL-6、IL-10、TNF-α,因变量为支气管扩张症患者肺功能受损,进行多因素Logisitic回归分析,结果显示,支气管扩张症患者肺功能受损的影响因素是FVC、FEV1/FVC、DLCO、ESR、CRP、WBC、NEUT、PCT、IL-4、IL-6、IL-10、TNF-α。结论:气道炎症随支气管扩张症患者FEV1水平升高而升高,其与肺功能密切相关,且是肺功能受损的影响因素。

      ABSTRACT Objective: This study was to investigate the correlation between airway inflammation and lung function impairment in patients with bronchiectasis with different FEV1 and to analyze the influencing factors of lung function impairment. Methods: 80 patients with bronchiectasis admitted to the respiratory department of our hospital from May 2022 to January 2024 were selected as research objects. General data of each group were analyzed, and lung function and airway inflammatory cytokine levels were compared in each group. Pearson linear analysis was used to analyze the relationship between lung function and airway inflammation in patients with bronchiectasis, and multi-factor Logisitic regression was used to analyze the influencing factors of lung function impairment in patients with bronchiectasis. Results: The FVC, FEV1/FVC and DLCO evaluation of the three groups were included in the study and the difference among the groups was compared. The results showed that the FVC, FEV1/FVC and FVC of the high FEV1 group and the medium FEV1 group were higher than those of the low FEV1 group. FVC, FEV1/FVC and FVC in high FEV11 group were higher than those in medium FEV1 group (P<0.05). The levels of ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6, IL-10 and TNF-α in the three groups were included in the study and the differences between the groups were compared. The levels of ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6 and TNF-α in high and medium FEV1 groups were lower than those in low FEV1 group, and the levels of IL-10 were higher than those in low FEV1 group (P<0.05). The levels of ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6 and TNF-α in high FEV1 group were lower than those in medium FEV1 group, and the levels of IL-10 were higher than those in medium FEV1 group(P<0.05). The results of Pearson linear analysis showed that FEV1, FVC, FEV1/FVC and DLCO were positively correlated with airway inflammatory factors ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6 and TNF-α, and negatively correlated with IL-10 in patients with bronchiectasis(P<0.05). Independent variables were set as the indicators with differences in comparison in the above data, including FVC, FEV1/FVC, DLCO, ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6, IL-10, and TNF-α. The dependent variable was impaired lung function in patients with bronchiectasis. Multivariate Logisitic regression analysis was performed, and the results showed that The influential factors of impaired lung function in patients with bronchiectasis are FVC, FEV1/FVC, DLCO, ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6, IL-10, TNF-α. Conclusion: Airway inflammation increases with the increase of FEV1 level in patients with bronchiectasis, which is closely related to lung function and is an influential factor of lung function impairment.
