| 摘要 目的：比较脾虚气陷证混合痔外剥内扎术患者采用不同频次电针足三里治疗的临床效果，旨在为脾虚气陷证混合痔外剥内扎术患者的康复提供临床指导。方法：选取2021年7月至2023年7月本院收治的脾虚气陷证混合痔外剥内扎术患者共计106例，以随机数字表法分成研究组（n=53）和对照组（n=53），两组均实施止血、抗炎、止痛药物等基础治疗，对照组于术前30 min给予电针足三里治疗，每日1次，研究组于术前30 min、术后2 h给予电针足三里治疗，每日2次，比较两组肛门疼痛评分、肛缘水肿评分、症状缓解时间、疼痛应激介质。结果：两组术后24 h肛门疼痛评分下降（P<0.05），研究组较对照组更低（P<0.05）；两组术后24 h肛缘水肿评分下降（P<0.05），研究组较对照组更低（P<0.05）；研究组症状（水肿、疼痛）缓解时间较对照组更短（P<0.05）；两组术后24 h前列腺素E2（PGE2）、P物质（SP）、皮质醇（COR）、促肾上腺皮质激素（ACTH）下降（P<0.05），研究组较对照组更低（P<0.05）。结论：脾虚气陷证混合痔外剥内扎术患者采用电针足三里治疗的效果较好，与每日电针足三里1次相比，每日电针足三里2次更有利于肛门疼痛和肛缘水肿评分、临床症状、疼痛应激介质的改善。 |

| ABSTRACT Objective: To compare the clinical effect of Zusanli electroacupuncture with different frequency in the treatment of patients with spleen deficiency qi depression syndrome mixed hemorrhoid external stripping and internal ligation surgery, and to provide clinical guidance for the rehabilitation of patients with spleen deficiency qi depression syndrome mixed hemorrhoid external stripping and internal ligation surgery. Methods: A total of 106 cases of patients with spleen deficiency qi depression syndrome mixed hemorrhoid external stripping and internal ligation surgery were selected and divided into study group (n=53) and control group (n=53) by random number table method. Both groups were given basic treatment with hemostatic, anti-inflammatory and analgesic drugs, while the control group was given Zusanli electroacupuncture 30 min before surgery, once a day. The study group was given Zusanli electroacupuncture 30min before surgery and 2 h after surgery, twice a day. The scores of anal pain score, anal edema score, symptom relief time and pain stress mediators were compared between the two groups. Results: The anal pain score of both groups decreased 24 h after operation (P<0.05), and the study group was lower than the control group(P<0.05). The anal edema score decreased 24 h after operation in both groups (P<0.05), and the score in the study group was lower than that in the control group(P<0.05). The symptom relief (edema, pain) time in the study group was shorter than that in the control group(P<0.05). Prostaglandin E2 (PGE2), Substance P(SP), cortisol(COR) and adrenocorticotropin(ACTH) decreased 24 h after operation in both groups(P<0.05), and the study group was lower than the control group (P<0.05). Conclusion: The effect of Zusanli electroacupuncture is better in patients with spleen deficiency qi depression syndrome mixed hemorrhoid external stripping and internal ligation surgery. Compared with Zusanli electroacupuncture once a day, Zusanli electroacupuncture twice a day is more beneficial to improve the scores of anal pain and anal edema, clinical symptoms and pain stress mediators. |
