摘要 目的:探讨羟苯磺酸钙胶囊联合全视网膜光凝术(PRP)对糖尿病视网膜病变(DR)患者视力状况、眼底微循环及脉络膜厚度的影响,以期为DR患者提供合理治疗方案。方法:选取2022年8月-2023年1月我院收治的的96例DR患者,按数字随机分组法分为试验组和对照组各48例,对照组采用PRP治疗,试验组采用羟苯磺酸钙胶囊联合PRP治疗,持续治疗4周。对比两组治疗总有效率和黄斑中心凹厚度(CMT)、最佳矫正视力(BCVA)、眼压水平以及眼底微循环[阻力指数(RI)、收缩期峰值平均流速(PSV)、舒张末期平均血流速度(EDV)]、脉络膜厚度[黄斑中心凹脉络膜厚度(SFCT)、上方脉络膜厚度(SCT)、下方脉络膜厚度(ICT)、颞侧脉络膜厚度(TCT)、鼻侧脉络膜厚度(NCT)]及并发症情况。结果:试验组治疗4周后的总有效率明显高于对照组(P<0.05)。治疗4周后两组DR患者CMT、BCVA、眼压水平较治疗前均显著改善,且试验组显著优于对照组(P<0.05)。治疗4周后两组DR患者EDV、PSV、RI水平较治疗前均显著改善,且试验组显著优于对照组(P<0.05)。治疗4周后两组DR患者SFCT、SCT、ICT、TCT、NCT水平较治疗前均显著改善,且试验组显著优于对照组(P<0.05)。两组DR患者治疗期间并发症总发生率相比无明显差异(P>0.05)。结论:羟苯磺酸钙胶囊联合PRP有利于提升DR患者的治疗总有效率,对改善患者视力状况、眼底微循环及脉络膜厚度具有积极作用,且安全性好,是临床治疗DR患者的合理有效的方案,为临床治疗DR提供了新的思路和方法。 |

ABSTRACT Objective: To investigate the effects of calcium dobesilate capsules combined with panretinal photocoa gulation(PRP) on visual acuity,fundus microcirculation and choroid thickness in patients with diabetic retinopathy (DR), in order to provide reasonable treatment for DR patients. Methods: A total of 96 patients with DR admitted to our hospital from August 2022 to January 2023 were selected and divided into experimental group and control group, 48 cases in each group, respectively. The control group was treated with PRP, and the experimental group was treated with calcium dobesilate capsules combined with PRP for 4 weeks. The total effective rate, central macular thickness (CMT), best corrected visual acuity (BCVA), intraocular pressure level, fundus microcirculation[resistance index (RI), peak systolic velocity(PSV), end diastolic velocity(EDV)], choroidal thickness[sub-foveal choroidal thickness (SFCT), and superior choroid thickness(SCT), inferior choroid thickness(ICT), temporal choroid thickness (TCT), nasal choroid thickness (NCT)] and complications were compared between the two groups. Results: After 4 weeks of treatment, the total effective rate of experimental group was significantly higher than that of control group(P<0.05). After 4 weeks of treatment, CMT, BCVA and intraocular pressure level of DR patients in both groups were significantly improved compared with before treatment, and the experimental group was significantly better than the control group(P<0.05). After 4 weeks of treatment, EDV, PSV and RI levels of DR Patients in both groups were significantly improved compared with before treatment, and the experimental group was significantly better than the control group (P<0.05). After 4 weeks of treatment, SFCT, SCT, ICT, TCT and NCT levels of DR Patients in both groups were significantly improved compared with before treatment, and the experimental group was significantly better than the control group(P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). Conclusion: Calcium dobesilate capsules combined with PRP is conducive to improving the total effective rate of DR patients, and has positive effects on improving patients' visual acuity, fundus microcirculation and choroidal thickness, and has good safety. It is a reasonable and effective scheme for clinical treatment of DR patients, and provides a new idea and method for clinical treatment of DR. |
