摘要 目的:研究慢性阻塞性肺疾病(COPD)合并肺癌患者血清长链非编码核糖核酸-人浆细胞瘤转化迁移基因1(LncRNA-PVT1)、微小核糖核酸-497-5p(miR-497-5p)表达与肺功能和预后的关系。方法:选择临沂市肿瘤医院2018年3月-2020年3月收治的116例COPD合并肺癌并行手术治疗的患者纳入观察组,同期单纯COPD患者50例纳入对照组。比较两组血清LncRNA-PVT1、 miR-497-5p相对表达水平以及肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC、第1秒用力呼气容积占预计值百分比(FEV1%pred)]。采用Pearson检验分析COPD合并肺癌患者血清LncRNA-PVT1、miR-497-5p表达与肺功能指标的相关性。COPD合并肺癌患者术后进行3年随访,采用多因素Cox回归模型分析COPD合并肺癌患者预后影响因素。绘制Kaplan-Meier生存曲线分析LncRNA-PVT1、miR-497-5p低表达和高表达患者3年总生存率情况。结果:与对照组比较,观察组血清LncRNA-PVT1表达水平显著升高(P<0.05),miR-497-5p表达水平显著降低(P<0.05)。与对照组比较,观察组FEV1、FEV1%pred、FEV1/FVC显著降低(P<0.05)。Pearson检验结果显示,COPD合并肺癌患者血清LncRNA-PVT1与FEV1、FEV1%pred、FEV1/FVC呈负相关(P<0.05);血清miR-497-5p与FEV1、FEV1%pred、FEV1/FVC呈正相关(P<0.05)。Kplan-Meier生存曲线显示,LncRNA-PVT1高表达组3年总生存率为29.23% 明显低于LncRNA-PVT1低表达组的47.73%(P<0.05);miR-497-5p高表达组3年总生存率为46.77%明显高于miR-497-5p低表达组的23.40%(P<0.05)。多因素Cox回归分析显示,淋巴结转移、TNM分期Ⅲa期、肿瘤组织低分化、LncRNA-PVT1高表达、miR-497-5p低表达是COPD合并肺癌患者预后的危险因素(P<0.05)。结论:COPD合并肺癌患者血清中LncRNA-PVT1呈高表达、miR-497-5p呈低表达,其表达水平与肺功能指标相关,且LncRNA-PVT1表达水平越高、miR-497-5p表达水平越低者的3年总生存率越低。 |

ABSTRACT Objective: To study the relationship between serum long chain non coding RNA-human plasmacytoma transformation and migration gene 1 (LncRNA-PVT1), microribonucleic acid-497-5p (miR-497-5p) expression and lung function and prognosis in patients with chronic obstructive pulmonary disease (COPD) with lung cancer. Methods: 116 patients with COPD combined with lung cancer underwent concurrent surgical treatment admitted to Linyi Cancer Hospital from March 2018 to March 2020 were selected as the observation group, while 50 patients with simple COPD during the same period were included in the control group. The relative expression levels of serum LncRNA-PVT1 and miR-497-5p and lung function indexes [forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), FEV1/FVC, percentage of forced expiratory volume in the first second to predicted value (FEV1%pred)] were compared between two groups. The correlation between serum LncRNA-PVT1, miR-497-5p expression and lung function indexes in patients with COPD with lung cancer were analyzed by Pearson test. Patients with COPD with lung cancer were followed up for 3 years after operation, the prognostic factors of patients with COPD with lung cancer were analyzed by multivariate Cox regression model. 3 years overall survival rate of patients with low expression and high expression of LncRNA-PVT1 and miR-497-5p were analyzed by drawn Kaplan-Meier survival curve. Results: Compared with control group, the expression level of serum LncRNA-PVT1 in observation group was significantly increased (P<0.05), and the expression level of miR-497-5p was significantly decreased (P<0.05). Compared with control group, FEV1, FEV1% pred and FEV1/FVC in observation group were significantly lower(P<0.05). Pearson test results showed that, serum LncRNA-PVT1 was negatively correlated with FEV1, FEV1% pred and FEV1/FVC in patients with COPD with lung cancer (P<0.05); serum miR-497-5p was positively correlated with FEV1, FEV1% pred and FEV1/FVC(P<0.05). The Kplan-Meier survival curve showed that, the 3 years overall survival rate in LncRNA-PVT1 high expression group was 29.23%, which was significantly lower than 47.73% in LncRNA-PVT1 low expression group(P<0.05). 3 years overall survival rate in miR-497-5p high expression group was 46.77%, which was significantly higher than 23.40% in miR-497-5p low expression group (P<0.05). Multivariate Cox regression analysis showed that, lymph node metastasis, TNM stage IIIa, poor differentiation of tumor tissue, high expression of LncRNA-PVT1 and low expression of miR-497-5p were risk factors for prognosis of patients with COPD with lung cancer (P<0.05). Conclusion: LncRNA-PVT1 is highly expresse and miR-497-5p is lowly expresse in the serum of patients with COPD with lung cancer, the expression level is relate to lung function indexes, and the higher the expression level of LncRNA-PVT1 and the lower the expression level of miR-497-5p, the lower the 3 years overall survival rate. |
