| 目的：探讨幽门螺杆菌（Hp）感染性胃癌组织原肌球蛋白1（TPM1）、Wiskott-Aldrich综合征蛋白家族成员3（WASF3）的表达及其与患者临床病理特征和预后的关系。方法：选择2016年3月至2018年3月南京市第二医院收治的163例Hp阳性胃癌患者（Hp阳性胃癌组）和117例Hp阴性胃癌患者（Hp阴性胃癌组）。取手术切除的Hp阳性胃癌组织和癌旁组织、Hp阴性胃癌组织和癌旁组织，采用免疫组化法检测TPM1、WASF3表达。比较不同临床病理特征Hp阳性胃癌组织中TPM1、WASF3阳性表达率的差异。Hp阳性胃癌患者随访5年，分析不同TPM1、WASF3表达Hp感染性胃癌患者生存差异和影响预后的相关因素。结果：Hp阳性胃癌组织TPM1阳性表达率低于Hp阴性胃癌组织、Hp阳性和HP阴性胃癌癌旁组织（P＜0.05），WASF3阳性表达率高于Hp阴性胃癌组织和Hp阳性和HP阴性胃癌癌旁组织（P＜0.05）。TPM1、WASF3表达与分化程度、浸润深度、TNM分期、淋巴结转移有关（均P＜0.05）。随访5年，患者死亡率为61.25%。TPM1阴性表达的Hp阳性胃癌患者5年累积生存率（OS）低于TPM1阳性表达的Hp阳性胃癌患者（P＜0.05），WASF3阳性表达的Hp阳性胃癌患者5年OS低于WASF3阴性表达的Hp阳性胃癌患者（P＜0.05）。淋巴结转移、TNM分期III期、WASF3阳性表达是Hp阳性胃癌患者预后不良的危险因素（P＜0.05），TPM1阳性表达是保护因素（P＜0.05）。结论：Hp阳性胃癌组织中TPM1阳性表达率降低，WASF3阳性表达率增高，与TNM分期 III期、淋巴结转移、浸润深度、低度分化和低生存率有关。 |

| Objective:To investigate the expression of tropomyosin 1 (TPM1) and Wiskott-Aldrich syndrome protein family member 3(WASF3) in gastric cancer tissue with helicobacter pylori (Hp) infection and their relationship with clinical pathological characteristics and prognosis.Methods:163 patients with HP-positive gastric cancer (HP-positive gastric cancer group) and 117 patients with HP-negative gastric cancer (HP-negative gastric cancer group) admitted to The Second Hospital of Nanjing from March 2016 to March 2018 were selected.Took the Hp-positive gastric cancer tissues and adjacent tissues, HP-negative gastric cancer tissues and adjacent tissues that had been surgically removed, the expressions of TPM1 and WASF3 were detected by immunohistochemical method. Compared the differences in the positive expression rates of TPM1 and WASF3 in HP-positive gastric cancer tissues with different clinicopathological features. Patients with HP-positive gastric cancer were followed up for 5 years, and analyzed the survival differences and related factors affecting prognosis in patients with HP-infected gastric cancer with different TPM1 and WASF3 expressions.Results:The positive expression rate of TPM1 in Hp positive gastric cancer tissues was lower than that in Hp negative gastric cancer tissues, Hp positive and Hp negative gastric cancer adjacent tissues (P<0.05), the positive expression rate of WASF3 was higher than Hp negative gastric cancer tissues and Hp positive and Hp negative gastric cancer adjacent tissues (P<0.05).The expression of TPM1 and WASF3 were correlated with differentiation degree, infiltration depth, TNM stage and lymph node metastasis (all P<0.05). After 5 year follow-up, the mortality rate was 61.25% of patients . The 5-year cumulative survival rate (OS) of HP-positive gastric cancer patients with negative expression of TPM1 was lower than that of HP-positive gastric cancer patients with positive expression of TPM1 (P<0.05), and the 5-year OS of HP-positive gastric cancer patients with positive expression of WASF3 was lower than that of HP-positive gastric cancer patients with negative expression of WASF3 (P<0.05).Lymph node metastasis,TNM stage III and WASF3 positive expression were risk factors for poor prognosis in HP-positive gastric cancer patients (P<0.05), and TPM1 positive expression was protective factor (P<0.05). Conclusion:The positive expression rate of TPM1 decrease and WASF3 increase in HP-positive gastric cancer tissues, which are related to TNM stage III, lymph node metastasis, depth of invasion, low differentiation and low survival rate. |
