      摘要 目的:探讨阿尔茨海默病(AD)患者血清淀粉样蛋白A(SAA)、胆碱酯酶(CHE)、载脂蛋白A1(ApoA1)水平与认知功能的关系。方法:选取2020年12月~2023年5月在我院就诊的103例AD患者作为病例组,另外选取同期在本院体检健康的志愿者80例作为对照组。采用酶联免疫吸附法检测两组研究对象血清SAA、CHE、ApoA1水平;采用蒙特利尔认知评估量表(MoCA)、简明精神状态检查量表(MMSE)评估两组研究对象的认知功能。对比两组研究对象血清SAA、CHE、ApoA1水平及MMSE、MoCA评分。根据MMSE评分将AD患者分为轻度组、中度组和重度组,观察并比较轻度组、中度组和重度组血清SAA、CHE、ApoA1水平。采用Pearson检验分析血清SAA、CHE、ApoA1水平与MMSE评分的相关性。结果:病例组的血清SAA高于对照组,血清CHE、ApoA1水平低于对照组(P<0.05)。病例组的MoCA、MMSE评分低于对照组(P<0.05)。重度组、中度组血清SAA水平高于轻度组,且重度组高于中度组(P<0.05)。重度组、中度组血清CHE、ApoA1水平低于轻度组,且重度组低于中度组(P<0.05)。Pearson检验分析结果显示,血清CHE、ApoA1和MMSE评分呈正相关,血清SAA和MMSE评分呈负相关(P<0.05)。结论:AD患者血清SAA异常升高,血清CHE、ApoA1水平异常下降,可能与认知功能障碍发生有关。

      ABSTRACT Objective: To investigate the relationship between serum amyloid A (SAA), cholinesterase (CHE), apolipoprotein A1 (ApoA1) levels and cognitive function in patients with Alzheimer's disease (AD). Methods: 103 AD patients who were treated in our hospital from December 2020 to May 2023 were selected as case group, and 80 healthy volunteers who underwent physical examination in our hospital during the same period were selected as control group. The levels of serum SAA, CHE and ApoA1 in two groups were detected by enzyme-linked immunosorbent assay. The cognitive function in two groups was evaluated by Montreal Cognitive Assessment (MoCA) and Mini-Mental State Examination (MMSE). Serum SAA, CHE, ApoA1 levels and MMSE, MoCA scores were compared between two groups. AD patients were divided into mild group, moderate group and severe group according to the MMSE score, the levels of serum SAA, CHE and ApoA1 in mild group, moderate group and severe group were observed and compared. The correlation between serum SAA, CHE, ApoA1 levels and MMSE score were analyzed by Pearson test. Results: The serum SAA in case group was higher than that in control group, and the serum CHE and ApoA1 levels were lower than those in control group(P<0.05). The scores of MoCA and MMSE in case group were lower than those in control group(P<0.05). The serum SAA levels in severe group and moderate group were higher than those in mild group, and severe group was higher than that of moderate group(P<0.05). The levels of serum CHE and ApoA1 in severe group and moderate group were lower than those in mild group, and severe group was lower than that of moderate group (P<0.05). Pearson test analysis showed that serum CHE, ApoA1 and MMSE scores were positively correlated, and serum SAA and MMSE scores were negatively correlated(P<0.05). Conclusion: The abnormal increase of serum SAA and the abnormal decrease of serum CHE and ApoA1 levels in AD patients may be relate to the occurrence of cognitive dysfunction.
