摘要 目的:探讨2型糖尿病心肌病(T2DCM)患者血清长链非编码核糖核酸 KCNQ1OT1(LncRNA KCNQ1OT1)、微小核糖核酸-181a-5p(miR-181a-5p)表达水平及其与左心功能的相关性。方法:选择2021年6月至2023年6月华中科技大学同济医学院附属同济医院收治的T2DCM患者80例作为T2DCM组,单纯2型糖尿病(T2DM)患者80例作为T2DM组,另选取同期体检健康者80例作为健康对照组。采用实时荧光定量聚合酶链式反应(qRT-PCR)检测并比较三组血清LncRNA KCNQ1OT1、miR-181a-5p表达水平。Pearson相关性分析法分析血清LncRNA KCNQ1OT1、miR-181a-5p表达与左心功能相关指标[二尖瓣早期血流速度峰值/左侧壁二尖瓣环早期峰值速度均值(E/e')、左心室舒张末期内径(LVEDD)、左室射血分数(LVEF)、Tei指数、左心室收缩末期内径(LVESD)、二尖瓣早期血流速度峰值/晚期血流速度峰值(E/A)]的相关性。结果:T2DCM组体质指数(BMI)、空腹血糖(FBG)、空腹胰岛素(FINS)、B型脑肽钠(BNP)、肌酸激酶(CK)、总胆固醇(TC)、糖化血红蛋白(HbAlc)、低密度脂蛋白胆固醇(LDL-C)、胰岛素抵抗指数(HOMA-IR)高于健康对照组(P<0.05);T2DCM组BNP、CK、TC、LDL-C高于T2DM组(P<0.05)。T2DCM组血清miR-181a-5p表达水平低于T2DM组、健康对照组(P<0.05);T2DCM组血清LncRNA KCNQ1OT1表达水平高于T2DM组、健康对照组(P<0.05)。T2DCM组LVEDD、LVESD、E/e'、Tei指数高于T2DM组、健康对照组(P<0.05),LVEF、E/A低于T2DM组、健康对照组(P<0.05)。T2DCM患者血清LncRNA KCNQ1OT1与LVEDD、LVESD、E/e'、Tei指数呈正相关(P<0.05),与E/A、LVEF、miR-181a-5p呈负相关(P<0.05);血清miR-181a-5p与LVEDD、LVESD、E/e'、Tei指数呈负相关(P<0.05),与E/A、LVEF呈正相关(P<0.05)。结论:T2DCM患者血清LncRNA KCNQ1OT1表达水平升高,miR-181a-5p表达水平降低,可能参与左心功能损伤过程。 |

ABSTRACT Objective: To investigate the expression levels of serum long non-coding RNA KCNQ1OT1 (LncRNA KCNQ1OT1) and microRNA-181a-5p (miR-181a-5p) in patients with type 2 diabetic cardiomyopathy (T2DCM) and their correlation with left ventricular function. Methods: 80 T2DCM patients who were admitted to Tongji Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from June 2021 to June 2023 were selected as T2DCM group, 80 patients with type 2 diabetes mellitus (T2DM) were selected as T2DM group, and 80 healthy patients with simultaneous physical examination were selected as healthy control group. The expression levels of serum LncRNA KCNQ1OT1 and miR-181a-5p in three groups were detected and compared by real-time fluorescence quantitative polymerase chain reaction (qRT-PCR). The correlation between serum LncRNA KCNQ1OT1, miR-181a-5p expression and left ventricular function [early mitral valve blood flow velocity peak/left wall mitral annulus early peak velocity mean (E/e'), left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), Tei index, left ventricular end-systolic diameter (LVESD), Peak blood flow velocity in the early mitral valve/late mitral valve blood flow velocity peak (E/A)] were analyzed by Pearson correlation analysis. Results: Body mass index (BMI), fasting blood glucose (FBG), fasting insulin (FINS), B type natriuretic peptide (BNP), creatine kinase (CK), total cholesterol (TC), glycosylated hemoglobin (HbAlc), low density lipoprotein cholesterol (LDL-C) and insulin resistance index (HOMA-IR) in T2DCM group were higher than those in healthy control group (P<0.05). BNP, CK, TC and LDL-C in T2DCM group were higher than those in T2DM group(P<0.05). The expression level of serum miR-181a-5p in T2DCM group was lower than that in T2DM group and healthy control group(P<0.05). The expression level of serum LncRNA KCNQ1OT1 in T2DCM group was higher than that in T2DM group and healthy control group(P<0.05). LVEDD, LVESD, E/e' and Tei index in T2DCM group were higher than those in T2DM group and healthy control group(P<0.05), LVEF and E/A were lower than those in T2DM group and healthy control group(P<0.05). Serum LncRNA KCNQ1OT1 in T2DCM patients was positively correlated with LVEDD, LVESD, Ee' and Tei index(P<0.05), and negatively correlated with E/A, LVEF and miR-181a-5p(P<0.05). Serum miR-181a-5p was negatively correlated with LVEDD, LVESD, E/e' and Tei index(P<0.05), and positively correlated with E/A and LVEF (P<0.05). Conclusion: The expression level of serum LncRNA KCNQ1OT1 in T2DCM patients is increase, and the decrease of miR-181a-5p expression level may be involve in the process of left ventricular dysfunction. |
