      摘要 目的:观察补肾化痰方联合穴位埋线对肾虚痰湿证多囊卵巢综合征(PCOS)患者性激素、糖脂代谢和外周血单核细胞Toll样受体4/核转录因子-κB(TLR4/NF-κB)信号通路的影响。方法:根据随机数字表法,将2019年10月-2021年10月期间来河北省沧州中西医结合医院就诊的80例肾虚痰湿证PCOS患者分为对照组(40例,常规西药治疗)和观察组(40例,补肾化痰方联合穴位埋线治疗)。治疗后,对比两组临床疗效,以及治疗前后的中医症候积分、性激素、糖脂代谢和外周血单核细胞TLR4/NF-κB信号通路相关指标。结果:观察组的临床总有效率高于对照组(P<0.05)。与对照组治疗后相比,观察组的中医症候总积分更低(P<0.05)。与对照组治疗后相比,观察组的促卵泡激素(FSH)、黄体生成素(LH)、总睾酮(T)水平更低,雌二醇(E2)水平更高(P<0.05)。与对照组治疗后相比,观察组的总胆固醇(TC)、三酰甘油(TG)、空腹胰岛素(FINS)、糖化血红蛋白(HbAlc)、胰岛素抵抗指数(HOMA-IR)水平均更低(P<0.05)。与对照组治疗后相比,观察组的TLR4、NF-κB、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α) mRNA均更低(P<0.05)。结论:补肾化痰方联合穴位埋线可有效改善肾虚痰湿证PCOS患者的性激素、糖脂代谢水平,可能通过抑制TLR4/NF-κB信号通路激活、减轻炎症反应来发挥作用。

      ABSTRACT Objective: To observe the effects of Bushen Huatan recipe combined with acupoint catgut embedding on sex hormone, glucose and lipid metabolism and Toll like receptor 4 / nuclear transcription factor-κB(TLR4/NF-κB) signal pathway of peripheral blood monocytes in patients with polycystic ovary syndrome (PCOS) with kidney deficiency and phlegm dampness syndrome. Methods: According to the random number table method, 80 patients with PCOS with kidney deficiency and phlegm dampness syndrome who came to Cangzhou Integrated Traditional Chinese and Western Medicine Hospital of Hebei Province from October 2019 to October 2021 were divided into control group (40 cases, treated with conventional western medicine) and observation group (40 cases, treated with Bushen Huatan recipe combined with acupoint catgut embedding). After treatment, the clinical effects, as well as the TCM syndrome score, sex hormone, glucose and lipid metabolism and TLR4/NF-κB signal pathway related indexes of peripheral blood monocytes before and after treatment of the two groups were compared. Results: The total clinical effective rate of the observation group was higher than that of the control group (P<0.05). Compared with the control group after treatment, the total score of TCM symptoms of the observation group was lower (P<0.05). Compared with the control group after treatment, the levels of follicle stimulating hormone (FSH), luteinizing hormone (LH), total testosterone (T) of the observation group were lower, and the level of estradiol (E2) was higher (P<0.05). Compared with the control group after treatment, the levels of total cholesterol (TC), triacylglycerol (TG), fasting insulin (FINS), glycosylated hemoglobin (HbAlc) and insulin resistance index (HOMA-IR) of the observation group were lower (P<0.05). Compared with the control group after treatment, TLR4, NF-κB, interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) mRNA of the observation group were lower (P<0.05). Conclusion: Bushen Huatan recipe combined with acupoint catgut embedding can effectively improve the levels of sex hormone, glucose and lipid metabolism in patients with PCOS with kidney deficiency and phlegm dampness syndrome, which possibly by inhibiting TLR4/NF-κB signaling pathway plays a role by activating and reducing inflammatory response.
