      摘要 目的:观察苍龟探穴电针疗法联合肩关节功能锻炼对肩周炎患者肩关节功能、炎症因子和血清5-羟色胺(5-HT)、前列腺素E2(PGE2)的影响。方法:选择2019年7月~2021年7月期间我院收治的肩周炎患者80例,按照入院的奇偶顺序分为对照组和研究组,各为40例,对照组接受肩关节功能锻炼,研究组接受苍龟探穴电针疗法联合肩关节功能锻炼,对比两组疗效、视觉疼痛模拟评分量表(VAS)评分、血清5- HT、PGE2水平、肩关节活动度(前屈、后伸、外展、内收、内旋、外旋)和肩关节周围肌力相关指标、血清炎症因子水平。结果:研究组的临床总有效率高于对照组(P<0.05)。治疗后,两组VAS评分均下降,且研究组较对照组低(P<0.05)。治疗后,两组主动和被动状态下患肢前屈、后伸、内收、外展、内旋、外旋活动度均改善,且研究组优于对照组(P<0.05)。治疗后,两组前屈峰力矩(PT)、前屈平均功率(AP)、外展PT、外展AP均升高,且研究组高于对照组(P<0.05)。治疗后,两组血清PGE2、5-HT水平均下降,且研究组低于对照组(P<0.05)。治疗后,两组血清白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平均下降,且研究组较对照组低(P<0.05)。结论:苍龟探穴电针疗法联合肩关节功能锻炼可促进肩周炎患者肩关节功能改善,减轻疼痛症状,作用机制可能与降低血清5- HT、PGE2及炎症因子水平有关。

      ABSTRACT Objective: To observe the effects of canggui acupoint electroacupuncture therapy combined with shoulder joint functional exercise on shoulder joint function, inflammatory factors, serum serotonin (5-HT) and prostaglandin E2 (PGE2) in patients with scapulohumeral periarthritis. Methods: 80 patients with scapulohumeral periarthritis who were received in our hospital from July 2019 to July 2021 were selected. The patients were divided into control group and study group according to the odd and even order of admission, with 40 cases in each group. The control group received shoulder joint functional exercise, and the study group received canggui acupoint electroacupuncture therapy combined with shoulder joint functional exercise. The curative effect, visual pain analogue scale (VAS) score, levels of serum 5-HT and PGE2, shoulder range of motion (forward flexion, backward extension, abduction, adduction, internal rotation, external rotation), related indexes of muscle strength around shoulder joint and the levels of serum inflammatory factors were compared between the two groups. Results: The total effective rate of the study group was higher than that of the control group (P<0.05). After treatment, the VAS score of the two groups decreased, and the VAS score of the study group was lower than that of the control group (P<0.05). After treatment, the activities of forward flexion, backward extension, adduction, abduction, internal rotation and external rotation of the affected limbs in the active and passive states of the two groups were improved, and the study group was better than the control group (P<0.05). After treatment, the peak moment (PT), average power (AP), abduction PT and abduction AP of the two groups increased, and the study group was higher than the control group(P<0.05). After treatment, the levels of serum PGE2 and 5-HT in the two groups decreased, and the study group was lower than the control group(P<0.05). After treatment, the levels of serum interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) in two groups decreased, and the study group was lower than the control group (P<0.05). Conclusion: Canggui acupoint electroacupuncture therapy combined with shoulder joint functional exercise can improve shoulder joint function in patients with scapulohumeral periarthritis, and relieve pain symptoms, and the mechanism of action may be related to the reduction of serum 5-HT, PGE2 and inflammatory factors.
