摘要 目的:探讨2型糖尿病(T2DM)合并代谢综合征(MS)患者血清25-羟维生素D[25(OH)D]、γ-谷氨酰转肽酶(γ-GGT)、尿酸/肌酐(SUA/SCr)比值与肥胖体表测量指标、糖脂代谢和胰岛素抵抗的相关性。方法:选取2017年1月~2021年12月我院收治的110例T2DM患者,根据是否合并MS分为MS组(n=49)和非MS组(n=61),检测两组血清25(OH)D、γ-GGT、SUA/SCr、肥胖体表测量指标、糖脂代谢和胰岛素抵抗指标并作组间对比,采用Pearson相关系数分析血清25(OH)D、γ-GGT、SUA/SCr与肥胖体表测量指标、糖脂代谢和胰岛素抵抗的相关性。结果:MS组的血清25(OH)D水平低于非MS组,而血清γ-GGT水平、SUA/SCr比值高于非MS组(P<0.05)。MS组的体质量指数(BMI)、腰围(WC)、腰臀比(WHR)均高于非MS组(P<0.05)。MS组的甘油三酯(TG)、空腹血糖(FBG)、胰岛素抵抗指数(HOMA-IR)、空腹胰岛素(FINS)水平均高于非MS组,而高密度脂蛋白胆固醇(HDL-C)水平低于非MS组,组间对比差异有统计学意义(P<0.05);两组间总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平差异无统计学意义(P>0.05)。Pearson相关性分析结果显示:T2DM合并MS患者的血清25(OH)D水平与BMI、WC、WHR、TG、FBG、FINS、HOMA-IR水平均呈负相关,而与HDL-C呈正相关(P<0.05);血清γ-GGT、SUA/SCr水平与BMI、WC、WHR、TG、FBG、FINS、HOMA-IR水平均呈正相关,而与HDL-C呈负相关(P<0.05)。结论:T2DM合并MS患者血清25(OH)D水平异常降低,γ-GGT、SUA/SCr水平异常升高,且与患者肥胖程度、糖脂代谢及胰岛素抵抗密切相关。 |

ABSTRACT Objective: To investigate the correlation of serum 25-hydroxyvitamin D[25 (OH) D], γ-glutamyltranspeptidase (γ-GGT), uric acid/creatinine ratio (SUA/SCr) with body surface measurement indexes of obesity, glucose and lipid metabolism and insulin resistance in patients with type 2 diabetes mellitus (T2DM) complicated with metabolic syndrome (MS). Methods: 110 patients with T2DM who were admitted to our hospital from January 2017 to December 2021 were selected, and they were divided into MS group (n=49) and non-MS group (n=61) according to whether or not MS was complicated. Serum 25(OH)D, γ-GGT, SUA/SCr, body surface measurement indexes of obesity, glucose and lipid metabolism, insulin resistance indexes were detected and compared between the two groups. Pearson correlation coefficient was used to analyze the correlation between serum 25(OH)D, γ-GGT, SUA/SCr and body surface measurement indexes of obesity, glucose and lipid metabolism and insulin resistance. Results: The serum 25(OH)D level in MS group was lower than that in non-MS group, while the serum γ-GGT level and SUA/SCr ratio were higher than those in non-MS group (P<0.05). The body mass index (BMI), waist circumference (WC) and waist-hip ratio (WHR) in MS group were higher than those in non-MS group (P<0.05). The triglyceride (TG), fasting blood glucose (FBG), insulin resistance index (HOMA-IR) and fasting insulin (FINS) in MS group were higher than those in non-MS group, while the high density lipoprotein cholesterol (HDL-C) level was lower than that in non-MS group, the differences were statistically significant (P<0.05). There were no significant differences in total cholesterol (TC) and low density lipoprotein cholesterol (LDL-C) between the two groups (P>0.05). Pearson correlation analysis showed that 25(OH)D level was negatively correlated with BMI, WC, WHR, TG, FBG, FINS, HOMA-IR, but positively correlated with HDL-C in patients with T2DM complicated with MS (P<0.05). Serum γ-GGT and SUA/SCr levels were positively correlated with BMI, WC, WHR, TG, FBG, FINS and HOMA-IR, while negatively correlated with HDL-C (P<0.05). Conclusion: Serum 25(OH)D level is abnormally decreased, and γ-GGT and SUA/SCr levels are abnormally increased in patients with T2DM complicated with MS, which are closely related to the degree of obesity, glucose and lipid metabolism and insulin resistance. |
