      摘要 目的:探讨急性髓系白血病(AML)患者血清可溶性E钙粘蛋白(SE-CAD)、可溶性程序性死亡因子配体-1 (sPD-L1)、巨噬细胞炎症蛋白-1α(MIP-1α)水平与临床疗效及预后的关系。方法:选取2018年1月~2020年1月蚌埠医学院第一附属医院收治的69例AML患者纳入AML组,另选同期在我院进行体检的健康志愿者50例为对照组。对所有受试者的SE-CAD、sPD-L1、MIP-1α水平进行检测对比。对AML组患者经标准化治疗后,进行为期9个月的随访观察,评估患者的疗效和预后,对比不同疗效及不同预后患者的血清SE-CAD、sPD-L1、MIP-1α水平。并采用Pearson检验对AML患者血清SE-CAD、sPD-L1、MIP-1α间的相关性进行分析。结果:AML组患者的血清SE-CAD、sPD-L1、MIP-1α水平均高于对照组(P<0.05)。在不同疗效患者对比中,未缓解组的血清SE-CAD、sPD-L1、MIP-1α水平高于部分缓解组和完全缓解组,且部分缓解组高于完全缓解组(P<0.05)。在不同预后患者对比中,死亡组的血清SE-CAD、sPD-L1、MIP-1α水平均高于存活组(P<0.05)。经Pearson检验分析,SE-CAD与sPD-L1、MIP-1α呈正相关性(r值=0.498、0.509,P值=0.006、0.002)。sPD-L1与MIP-1α呈正相关性(r值=0.517,P值=0.000)。结论:AML患者血清SE-CAD、sPD-L1、MIP-1α水平异常升高,且观察以上指标水平对患者疗效及预后评估有一定指导作用,可能作为AML诊断、疗效及预后评估的辅助指标。

      ABSTRACT Objective: To investigate the relationship between levels of serum soluble E-cadherin (SE-CAD), soluble programmed death factor ligand-1 (sPD-L1) and macrophage inflammatory protein-1α (MIP-1α) and their curative effect and prognosis in patients with acute myeloid leukemia (AML). Methods: 69 patients with AML who were admitted to The First Affiliated Hospital of Bengbu Medical College from January 2018 to January 2020 were included in the AML group, and 50 healthy volunteers who underwent physical examination in our hospital during the same period were selected as the control group. The levels of SE-CAD, sPD-L1 and MIP-1α of all subjects were compared. Patients in the AML group were followed up for 9 months after standardized treatment, the curative effect and prognosis of the patients were evaluated, and the levels of serum SE-CAD, sPD-L1 and MIP-1α in patients with different curative effect and different prognosis were compared. The correlation among serum SE-CAD, sPD-L1 and MIP-1α in patients with AML was analyzed by Pearson test. Results: The levels of serum SE-CAD, sPD-L1 and MIP-1α in AML group were higher than those in the control group(P<0.05). In the comparison of different curative effect patients, the levels of serum SE-CAD, sPD-L1 and MIP-1α in the non-remission group were higher than those in the partial remission group and complete remission group, and those in the partial remission group were higher than those in the complete remission group(P<0.05). In the comparison of different prognosis patients,the levels of serum SE-CAD, sPD-L1 and MIP-1α in the death group were higher than those in the survival group (P<0.05). After Pearson test and analysis, SE-CAD was positively correlated with sPD-L1 and MIP-1α (r value=0.498, 0.509, P value=0.006, 0.002), and there was a positive correlation between sPD-L1 and MIP-1α (r value=0.517, P value=0.000). Conclusion: The levels of serum SE-CAD, sPD-L1 and MIP-1α in patients with AML are abnormally increased.The observation of the above indexes has a certain guiding role in the evaluation of curative effect and prognosis of patients, and which may be used as auxiliary indexes for the diagnosis, curative effect and prognosis of AML.
