      摘要 目的:探讨慢性阻塞性肺疾病(COPD)患者血清8-异前列腺素F2α(8-iso-PGF2α)、低氧诱导因子(HIF-1α)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、半胱氨酸天冬酶-3(caspase-3)水平与肺功能及认知功能障碍的关系。方法:选取2018年3月至2020年6月期间我院收治的62例COPD合并认知功能障碍患者(认知功能障碍组),另选取50例COPD未合并认知功能障碍患者为对照组。比较两组血清8-iso-PGF2α、HIF-1α、NGAL、caspase-3水平和肺功能、蒙特利尔认知评估量表(MoCA)评分,分析COPD合并认知功能障碍患者血清8-iso-PGF2α、HIF-1α、NGAL、caspase-3与肺功能、MoCA评分的相关性。结果:认知功能障碍组血清8-iso-PGF2α、HIF-1α、NGAL、caspase-3水平高于对照组(P<0.05),认知功能障碍组第1秒用力呼气容积(FEV1)、FEV1与用力肺活量比值(FEV1/FVC)、 FEV1占预计值百分数(FEV1%pred)、MoCA评分均低于对照组(P<0.05)。Pearson相关分析结果显示COPD合并认知功能障碍患者血清8-iso-PGF2α、HIF-1α、NGAL、caspase-3水平与FEV1、FEV1/FVC、FEV1%pred、MoCA评分均呈负相关(P<0.05)。结论:COPD合并认知功能障碍患者血清8-iso-PGF2α、HIF-1α、NGAL、caspase-3水平较COPD未合并认知功能障碍患者明显升高,8-iso-PGF2α、HIF-1α、NGAL、caspase-3升高与肺功能下降和认知功能受损均存在相关性,对COPD患者病情及认知功能障碍具有一定预测价值。

      ABSTRACT Objective: To investigate the relationship between serum level of 8-isoprostaglandin F2 (8-iso-PGF2α), hypoxia-inducing factor (HIF-1α), neutrophil gelatinase-associated lipid carrier protein (NGAL), and caspase-3 (caspase-3) and lung function and cognitive impairment in patients with chronic obstructive pulmonary disease (COPD). Methods: 62 patients with COPD combined with cognitive impairment (cognitive impairment group) who were admitted to our hospital from March 2018 to June 2020 were selected, another 50 COPD patients without cognitive impairment were selected as control group. Serum levels of 8-iso-PGF2α, HIF-1α, NGAL, caspase-3 and lung function, Montreal Cognitive Assessment (MoCA) score of the two groups were detected, and the correlation between 8-iso-PGF2α, HIF-1α, NGAL, caspase-3 and lung function, MoCA score,were analyzed. Results: The serum levels of 8-iso-PGF2α, HIF-1α, NGAL, caspase-3 in the cognitive impairment group were higher than those of the control group (P<0.05), and the first second forced expiratory volume (FEV1), FEV1 to forced vital capacity ratio (FEV1/FVC) , FEV1% of predicted value(FEV1%pred), MoCA score in the cognitive impairment group were all lower than those of the control group (P<0.05). Pearson correlation analysis showed that serum levels of 8-iso-PGF2α, HIF-1α, NGAL, caspase-3 were negatively correlated with FEV1, FEV1/FVC, FEV1% pred, MoCA score in COPD patients with cognitive impairment (P<0.05). Conclusion: Serum levels of 8-iso-PGF2α, HIF-1α, NGAL, caspase-3 in COPD patients with cognitive impairment are significantly higher than the patients without cognitive impairment, the increase of 8-iso-PGF2α, HIF-1α, NGAL, caspase-3 are correlated with the decline of lung function and cognitive impairment, which can provide reference for predictive value of COPD patients with state of an illness and cognitive impairment.
