摘要 目的:研究玉泉汤加减对口腔黏膜下纤维化(oral submucous fibrosis,OSF)癌变的干预效果及对原癌基因(c-Myc)、细胞周期蛋白D1(Cyclin D1)、β-连环蛋白(β-catenin)表达的影响。方法:本研究选取2018年4月至2019年4月在我院就诊的OSF癌变患者92例,按照完全随机法将患者分为西药组和中药组,每组各46例。西药组给予曲安奈德注射液联合盐酸利多卡因注射,中药组在西药组的基础上加服中药玉泉汤加减,检测和比较两组患者治疗前后β-catenin、纤维蛋白原(FIB)、c-Myc、白介素-6(IL-6)、转化生长因子-β1(TGF-β1)、Cyclin D1表达的变化,治疗效果以及不良反应的发生情况。结果:中药组治疗后的总有效率(91.30%)显著高于西药组(67.39%,P<0.05)。两组患者治疗后β-catenin、FIB、c-Myc、IL-6、TGF-β1及Cyclin D1的表达均低于治疗前(P<0.05);中药组治疗后β-catenin、FIB、c-Myc、IL-6、TGF-β1及Cyclin D1的表达均明显低于西药组(P<0.05)。中药组治疗后开口度比西药组小、灼痛感轻及纤维条索少(P<0.05)。中药组治疗后的不良反应率(2.17%)显著低于西药组(13.04%,P<0.05)。结论:玉泉汤加减治疗OSF癌变的效果及安全性均显著优于曲安奈德注射液联合盐酸利多卡因注射治疗,可能与其能显著下调c-Myc、Cyclin D1、β-catenin的表达,改善患者的临床症状有关。 |

ABSTRACT Objective: To study the intervention effect of Yuquan Decoction on OSF carcinogenesis and the expression of c-Myc protein, cyclin D1 and β-catenin. Methods: In this study, 92 patients with OSF canceration who were treated in our hospital from April 2018 to April 2019 were divided into western medicine group and traditional Chinese medicine group according to the complete random method, 46 cases in each group. Western medicine group was given triamcinolone acetonide injection plus lidocaine hydrochloride injection. The traditional Chinese medicine group was given Yuquan Decoction on the basis of Western medicine group. The changes of β-catenin, FIB, c-Myc, IL-6, TGF-β1 and Cyclin D1 were detected and compared before and after treatment. And the therapeutic effect and adverse reactions of the two groups were compared. Results: After treatment, the total effective rate (91.30%) of Chinese medicine group was significantly higher than that of Western medicine group (67.39%, P<0.05). After treatment, the expressions of β-catenin, FIB, c-Myc, IL-6, TGF-β1 and Cyclin D1 in the two groups were lower than those before treatment (P<0.05); after treatment, the expressions of β-catenin, FIB, c-Myc, IL-6, TGF-β1 and Cyclin D1 in the traditional Chinese medicine group were significantly lower than those in the western medicine group (P<0.05). After treatment, the opening degree of the Chinese medicine group was smaller, the burning pain was lighter and the fibrous strips was less than those of the western medicine group (P<0.05). The adverse reaction rate (2.17%) in the Chinese medicine group was significantly lower than that in the western medicine group (13.04%, P<0.05). Conclusion: The efficacy and safety of Yuquan Decoction in treating OSF canceration were significantly better than that of triamcinolone acetonide injection combined with lidocaine hydrochloride injection, which may be related to its significant down-regulation of c-Myc, Cyclin D1 and beta-catenin expression and improvement of clinical symptoms. |
