      摘要 目的:探讨彩色多普勒超声显示肝硬化上消化道出血患者门静脉的诊断价值。方法:选取2015年9月~2016年9月于我院就诊的肝硬化上消化道出血病患50例为研究组。另选取50名健康者为正常对照组,应用彩色多普勒超声诊断仪对两组患者的左侧门静脉内径 (Diameter of left portal vein,Dpv)、门静脉血流量(Portal vein flow,Pvf)、门静脉平均血流速度(Velocity of blood portal vein,Vpv)、右侧脾静脉内径(Diameter splenic vein,Dsv)、脾静脉血流量(Splenic vein flow,Svf)、脾静脉平均血流速度(Splenic portal blood flow velocity,Spv)进行检测。结果:与正常对照组相比,研究组Dpv扩大,Pvf明显增多,Vpv则减慢,研究组门静脉各项检测指标与对照相比,差异显著具有统计学意义(t=8.782、5.428、6.382,P=0.001、0.026、0.013)。研究组的Dsv、Svf明显高于对照组, Spv显著低于对照组,组间比较差异显著有统计学意义(t=5.683、3.243、3.365,P=0.018、0.037、0.034)。结论:运用彩色多普勒超声检测肝硬化病人门静脉血流动力学可有效判断病情进展情况及治疗方案的指导。

      ABSTRACT Objective: To evaluate the diagnostic value of color Doppler ultrasonography in the portal vein of patients with liver cirrhosis and upper gastrointestinal bleeding. Methods: Fifty cases of patients with liver cirrhosis and upper gastrointestinal bleeding treated in our hospital from September, 2015 to September, 2016 were selected as the study group, and other 50 healthy subjects were enrolled in the control group. The color Doppler ultrasonography was used to detect the left portal vein diameter(Dpv), portal vein blood flow (Pvf), the average blood flow velocity of portal vein (Vpv), right diameter of splenic vein (Dsv), splenic vein flow (Svf), the average splenic portal blood flow velocity (Spv) of two groups. Results: Compared with the normal control group, the Dpv was expanded and the Pvf was increased significantly while the Vpv was decreased in the study group, and the differences in the portal vein indexes in the study group was statistically significant compared with those in the control group (t=8.782, 5.428, 6.382, P=0.001, 0.026, 0.013). The levels of Dsv and Svf in the study group were significantly higher than those in the control group while the Spv level was significantly lower than that in the control group (t=5.683, 3.243, 3.365, P=0.018, 0.037, 0.034). Conclusion: The color Doppler ultrasonography in the detection of portal vein hemodynamics in patients with liver cirrhosis can effectively determine the progress of the disease and treatment program guidance.
