| 摘要 目的：研究血必净注射液联合奥曲肽用于急性重症胰腺炎的临床疗效及对血清肿瘤坏死因子α(TNF-α)、高迁移率族蛋白1(HMGB1)、闭锁小带蛋白1(ZO-1)水平的影响。方法：选择2013年11月至2016年11月在我院接受治疗的急性重症胰腺炎患者116例，按照不同治疗方法分为对照组和观察组，对照组使用奥曲肽治疗，观察组使用血必净联合奥曲肽治疗。比较两组患者临床疗效及治疗前后血清TNF-α、HMGB1、ZO-1、D-乳酸、内霉素、二胺氧化酶水平、乳果糖/甘露醇排除比值的变化及治疗后临床症状缓解时间和器官功能衰竭的发生情况。结果：与治疗前相比，患者血清TNF-α、HMGB1、D-乳酸、内霉素、二胺氧化酶、乳果糖/甘露醇排除比值水平均显著降低，血清ZO-1水平有所提高(P<0.05)；与对照组相比，观察组总有效率(93.1%)较高，血清TNF-α、HMGB1、D-乳酸、内霉素、二胺氧化酶水平、乳果糖/甘露醇排除比值较低，血清ZO-1蛋白表达较高，临床症状缓解时间较短，器官功能衰竭的发生率较低(P<0.05)。结论：血必净注射液联合奥曲肽治疗急性重症胰腺炎能有效改善患者的临床症状及肠道黏膜屏障损伤，提高临床疗效，可能与其降低患者血清TNF-α、HMGB1水平，提高血清ZO-1表达有关。 |

| ABSTRACT Objective: To study the curative efficacy of Xuebijing injection combined with octreotide in the treatment of acute se- vere pancreatitis and its effects on the serum tumor necrosis factor-α(TNF-α), high mobility group protein 1 (HMGB1), small atresia with protein 1 (ZO -1) levels. Methods: 116 patients with acute severe pancreatitis who were treated from November 2013 to November 2016 in our hospital were selected as the research objects and divided into the control group and the observation group according to dif- ferent treatments. The control group was given octreotide therapy, while the observation group was given Xuebijing injection combined with octreotide therapy. The clinical curative effect and changes of serum TNF-α, HMGB1, ZO-1, D-lactic acid, doxycycline, diamine oxidase levels, lactulose/mannitol ratio before and after treatment, the relieving time of clinical symptoms after treatment, incidence of or- gan failure were compared between two groups. Results: Compared with before treatment, the serum TNF-α, HMGB1, D-lactic acid, doxycycline, diamine oxidase, lactulose/mannitol ratio levels in both groups were significantly reduced, the serum ZO-1 levels were in- creased(P < 0.05). Compared with the control group, the total effective rate (93.1%) of observation group was higher, the serum TNF-α, HMGB1, D-lactic acid, diamine oxidase, lactulose/mannitol ratio were obviously lower, the serum ZO-1 level was higher, the relieving time of clinical symptoms time was shorter, the incidence of organ failure was lower (P<0.05). Conclusion: Xuebijing injection combined with octreotide could effectively improve the clinical symptoms and the intestinal mucosa barrier damage, improve the clinical curative effect in the treatment of acute severe pancreatitis, it might be related to the reduction of serum TNF-α, HMGB1 levels, increase the serum ZO-1 level. |
