摘要 目的:研究血浆脂蛋白磷脂酶A2(Lp-PLA2)、同型半胱氨酸(Hcy)与急性脑梗死患者病情严重程度及血脂水平的相关性。方法:选取2016年5月至2017年6月我院收治的急性脑梗死患者140例为观察组,同期健康体检者140例为对照组。分别检测两组研究对象的血浆Lp-PLA2、Hcy以及血脂指标水平,并作相关性分析。根据观察组患者神经功能缺损程度评分(NIHSS)并将其分为轻型组60例,中型组55例以及重型组25例,对比三组患者的血浆Lp-PLA2、Hcy水平。结果:观察组血浆Lp-PLA2、Hcy水平以及甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平较对照组高,而高密度脂蛋白胆固醇(HDL-C)水平较对照组低,差异均有统计学意义(均P<0.05)。经Pearson相关性分析可得:血浆Lp-PLA2、Hcy与HDL-C均呈负相关关系(r=-0.593、-0.701,均P<0.05),而与LDL-C均呈正相关关系(r=0.576、0.672,均P<0.05)。重型组、中型组患者Lp-PLA2、Hcy水平显著高于轻型组,同时重型组患者Lp-PLA2、Hcy水平显著高于中型组,差异均有统计学意义(均P<0.05)。结论:血浆Lp-PLA2、Hcy水平与急性脑梗死患者的HDL-C均呈负相关关系,与LDL-C均呈正相关关系。且随着患者病情越严重,血浆Lp-PLA2、Hcy水平越高。 |

ABSTRACT Objective: To study the correlation analysis between plasma lipoprotein phospholipase A2(Lp-PLA2), homocysteine(Hcy)and the severity and the level of blood lipid in the patients with acute cerebral infarction. Methods: A total of 140 patients with acute cerebral infarction, who were treated in Taizhou People's Hospital from May 2016 to June 2017, were chosen as observation group; 140 healthy subjects were enrolled as control group at the same time. The levels of plasma Lp-PLA2, Hcy and serum lipids in the two groups were detected respectively and made a correlation analysis. The observation group were further divided into light group(n=60), medium size group(n=55) and heavy group(n=25) according to the degree of neurologic impairment (NIHSS). The levels of plas- ma Lp-PLA2 and Hcy were compared among the three groups. Results: The levels of plasma Lp-PLA2, Hcy, triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) of the observation group were higher than those of the control group, the level of high-density lipoprotein cholesterol (HDL-C) was lower than that of the control group, the differences were statistically significant (all P<0.05). Pear- son correlation analysis showed that the levels of plasma Lp-PLA2 and Hcy were negatively correlated with HDL-C (r=-0.593, -0.701, all P<0.05), but positively correlated with LDL-C (r=0.576, 0.672, all P<0.05). The levels of Lp-PLA2 and Hcy in the heavy group and medium-sized group were significantly higher than those in the light group, the Lp-PLA2 and Hcy levels in the heavy group were signifi- cantly higher than those in the medium-sized group, the differences were statistically significant (all P<0.05). Conclusion: The levels of plasma Lp-PLA2 and Hcy were negatively correlated with HDL-C in the patients with acute cerebral infarction, and positively correlated with LDL-C. The more severe the patient's condition, the higher the levels of plasma Lp-PLA2 and Hcy. |
