目的:研究关节镜手术在膝关节骨性关节炎患者中的综合应用,为膝关节骨性关节炎的临床治疗提供依据。方法:选取50 例
单膝关节骨性关节炎患者,行常规关节镜手术,术后注射透明质酸钠并指导患者进行康复训练。在关节镜手术前后对患者进行
LKSS 评分,评定患者膝关节功能;对患膝的屈肌、伸肌力进行等速测定;在术前及术后2 周测定外周血及患膝关节液IL-6、
TNF-alpha、MMP-3表达水平。结果:与术前相比,患者术后LKSS 评分及关节活动度均显著提高(P<0.05),关节功能得到较好的恢
复;患膝伸肌PT、STW 及TAE 值均得到显著提高(P<0.05),屈肌PT、STW 及TAE 值无统计学意义;患者外周血及关节液中
IL-6、TNF-alpha及MMP-3 表达水平均显著降低(P<0.05)。结论:采用关节镜手术治疗骨性关节炎可明显降低机体的炎症状态,改善
关节的功能,增加患膝的伸肌力。 |

Objective:To study the comprehensive application of arthroscopic surgery in patients with knee osteoarthritis in order
to provide the basis for the clinical treatment of knee osteoarthritis.Methods:50 cases of knee arthritis single bone were chosen, and they
were taken with routine arthroscopic surgery. They were taken with injection of sodium hyaluronate and guidance of rehabilitation after
surgery. The patients were taken with LKSS scores before and after arthroscopic surgery to assess knee function. The flexor and extensor
force were taken for measurement. The IL- 6, TNF-alpha, MMP-3 expression levels of peripheral blood and fluid before surgery and two
weeks after treatment were taken for determination.Results:Compared with before surgery, the LKSS score and range of motion after
surgery in the two groups significantly increased, and the differences were statistically significant (P<0.05). The joint function had gotten
a better recovery. The knee extensor PT, STWand TAE values significantly increased (P < 0.05); There was no statistically significant
about the flexor PT, STW and TAE (P>0.05); The peripheral blood and synovial fluid IL-6, TNF-alpha and MMP-3 expression levels
significantly decreased, and the differences were statistically significant (P<0.05).Conclusion:The arthroscopic surgery for osteoarthritis
can significantly reduce the body's inflammation and improve joint function and increase the risk of knee extensor force. |
