      目的:探讨高渗氯化钠联合纳洛酮对失血性休克患者的临床疗效及安全性。方法:收集我院收治的72 例创伤失血性休克患 者,随机分为实验组和对照组,每组36 例,两组入院后均给予ICU 常规治疗,对照组患者给予纳洛酮注射液1.2 mg 加入5%葡萄 糖静注,5 min/次,3 次后改用4 mg持续静滴;实验组患者在对照组的基础上给予10%氯化钠液220 mL 加入生理盐水80 mL配 成7.5%的高渗氯化钠溶液静脉滴注。治疗结束后,对两组患者苏醒时间、血清NO、CD18 水平以及临床疗效进行检测和比较。结 果:与治疗前相比,两组患者的血清NO、CD18 水平均下降(P<0.05);与对照组相比,实验组患者的苏醒时间较短,血清NO、CD18 水平较低,并发症的发生率也较低(P>0.05)。结论:高渗氯化钠联合纳洛酮能够提高失血性休克患者的临床疗效,且安全性高,可 能与其降低失血性休克患者血清NO、CD18 水平有关。

      Objective:To investigate the effect and safety of hypertonic sodium chloride combined with naloxone on patients with hemorrhagic shock.Methods:72 cases of patients with traumatic hemorrhagic shock admitted in our hospital were selected and randomly divided into the experimental group and control group with 36 cases in each group. Both groups were given ICU routine treatment after admission, patients in the control group received naloxone injection 1.2 mg adding 5%glucose intravenous injection, once/5 min, after 3 times of switch to 4 mg by intravenous drip; patients in the experimental group were treated on the basis of the control group with 10% sodium chloride solution 220 mL saline 80 ml with 7.5% hypertonic sodium chloride solution by intravenous drip. After the end of treatment, the recovery time of patients, serum NO, CD18 levels and clinical efficacy of patients were detected and compared.Results:Compared with before treatment, the serumnitric oxide (no) and CD18 levels of both groups were decreased (P <0.05); compared with the control group, the recovery time of experimental group was shorter, serumnitric oxide (no) and CD18 levels as well as the incidence of complications were lower (P>0.05).Conclusion:Hypertonic sodium chloride combined with naloxone could enhance the clinical effect of patients with traumatic hemorrhagic shock with high safety, which might be related to the decrease of serumNO, CD18 levels.
