法:收集我院收治的172 例老年STEMI 患者,按照住院期间是否发生HF 分为HF 组(n=55 例)和非HF 组(n=117 例),以患者
MPV 四分位分四组,比较以上各组之间相关指标的差异,用Logistic 回归方程分析MVP 与患者HF 发生的关系。结果:HF组与
非HF 组在吸烟、发病至入院时间、前壁梗死、血清B 型脑钠肽(BNP)、肌钙蛋白I(cTnI)、左室射血分数(LVEF)、MVP 存在统计学
差异(P<0.05),HF 组MVP水平高于非HF 组(P<0.05);MVP 四分位分组之间的心功能指标LVEF 和血清BNP、HF发生率存在
统计学差异(P<0.05),MVP 的第四四分位组(Q4 组)的HF发生率高于第一分位组及第二四分位组(Q1 及Q2 组);多因素Logistic
回归方程分析显示高水平MPV是老年STEMI 患者近期发生心力衰竭的独立危险因素(P<0.05);MVP 四分位分组中,从Q1 组
到Q4 组发生HF 的风险值(OR)依次增高,且Q3 及Q4 组的OR值具有统计学意义(P<0.05)。结论:高水平MPV 与老年STEMI
患者住院期间HF发生密切相关,可能是其发生的独立危险因素,应当引起临床关注。 |

Objective:To investigate the relationship between mean platelet volume (MPV) and in-hospital heart failure (HF) of
the elderly patients with acute ST-segment elevation myocardial infarction (STEMI).Methods:172 cases of elderly patients with STEMI
were sleeted as research subjects. All cases were divided into two groups: HF group (n = 55cases) and non-HF group (n =117), and all
cases were also divided into four groups according to quartile. The differences of observation index between each group were compared,
and the relationship between mean platelet volume (MPV) and in-hospital heart failure were analyzed by Logistic regression analysis.Results:There were statistical differences in smoking, onset to admission time, anterior wall infarction, serum BNP, cTnI, LVEF, MVP
between HF and non-HF group (P<0.05). The MVP level of HF group was higher than that of non-HF group (P<0.05). There were also
statistical differences in LVEF, serum BNP and incidence of HF among the MVP quarterback groups (P<0.05). The HF incidence of Q4
group was higher than that of Q1 and Q2 group (P<0.05). Multivariable Logistic regression equation analysis showed high level of MPV
was independent risk factor for in-hospital heart failure of the elderly patients with STEMI. The risk of in-hospital HF increased from Q1
to Q4 group, and the risk of Q3 and Q4 group had statistically significant difference (P<0.05).Conclusion:High level of MPV is closely
related to in-hospital heart failure of the elderly patients with STEMI, and it may be independent risk factor for it. Clinicians should pay
much attention to MPV. |
