目的:研究突发性耳聋患者(SD)治疗前后血脂及血液流变学水平变化及临床意义。方法:选择2012 年3 月至2015 年3 月
在我院接受治疗的SD患者106 例(106 耳)进行研究,使用药物治疗后,根据疗效情况进行分组,主要分成观察组(治疗有效者)
及对照组(治疗无效者)各53 例(53 耳)。对比治疗前后两组的血脂及血流变水平,并分析治疗前后两组与患者听阈的相关性。结
果:观察组治疗后的TG、TC 及LDL-C水平均分别显著低于对照组,HDL-C 水平显著高于对照组(均P<0.05)。治疗后观察组的
高切粘度、中切粘度、低切粘度、红细胞聚集指数均显著低于对照组,红细胞变形指数显著高于对照组(均P< 0.05)。治疗后观察
组的听阈显著低于对照组(P< 0.05)。治疗前听阈与HDL-C 呈负相关,与低切粘度及血浆粘度呈正相关,而治疗后听阈与血脂指
标及血流变指标均无明显相关性(P>0.05)。结论:SD 患者治疗前血脂及血流变指标均处于异常状态,且听阈与HDL-C 呈负相关,
与低切粘度及血浆粘度呈正相关,对上述指标的监测有利于临床SD 诊断及治疗。 |

Objective:To study the changes of serum lipids and hemorheology levels in patients with sudden deafness(SD) and its
clinical significance.Methods:Selected 106 cases (106 ears) of patients with SD were accepted treatment in our hospital from March
2012 to March 2015 as study objects, who were divided into observation group(valid treatment) and control group(invalid treatment) according
to the medicine treatment effect, with 53 patients (53 ears) in each group. Compared the serumlipids and hemorheology levels in
the two groups before and after treatment,and analyzed their correlation with auditory threshold before and after treatment.Results:The
TG,TC and LDL-C levels of observation group were all significantly lower than control group,and the HDL-C level was significantly
higher than control group (P<0.05). The high shear viscosity, middle shear viscosity, low shear viscosity and red cell assembling index in
the observation group after treatment were significantly lower than control group, while the erythrocyte deformation index was significantly
higher than control group (P<0.05). The auditory threshold in observation group was significantly lower than control group after
treatment(P<0.05). The auditory threshold was negatively correlated with HDL-C level, and was positively correlated with low shear viscosity
and serum viscosity before treatment,w hile the auditory threshold had no significant correlation with serum lipids and hemorheology
indexes after treatment (P>0.05).Conclusion:The serum lipids and hemorheology indexes of patients with SD are in abnormal state
before treatment, and the auditory threshold is negatively correlated with HDL-C level, and positively correlated with low shear viscosity
and serumviscosity, which indicate that monitoring above indexes is helpful for clinical SD diagnosis and treatment. |
