      目的:观察分析胃癌螺旋CT 表现与组织蛋白酶D(Cath-D)蛋白和基质金属蛋白酶-9(MMP-9)表达的关系。方法:采集自 2014 年9月至2015 年9 月在医院经过胃癌手术治疗的54 例患者自身胃癌组织标本进行本次研究。再选同期在医院就诊并经过 手术治疗的54 例胃溃疡患者自身胃黏膜组织进行对照。分别检测MMP-9 以及Cath-D 蛋白的表达,对所有胃癌患者给予螺旋 CT 诊断。结果:MMP-9 及Cath-D 蛋白在胃癌组织内的阳性表达率为74.07%及77.79%,显著高于在正常胃黏膜内的5.56%及 9.26%,差异均有统计学意义(P<0.05)。MMP-9 及Cath-D表达存在明显正相关(r=0.693,P<0.05)。胃癌组织内有淋巴结转移及 Lauren 分型为弥漫型的MMP-9及Cath-D 阳性表达率均显著高于无淋巴结转移及Lauren分型为肠型者,差异有统计学意义(P< 0.05)。根据Logistic 回归分析可知,有淋巴结转移以及Lauren 分型为弥漫型是胃癌组织内MMP-9 及Cath-D 表达的螺旋CT 表 现相关因素。结论:胃癌患者组织内的MMP-9 及Cath-D 表达比正常胃黏膜组织更高,MMP-9 及Cath-D 二者的表达呈现正相关, 且有淋巴结转移以及Lauren 分型为弥漫型是胃癌组织内MMP-9 及Cath-D 表达的螺旋CT 表现相关因素。

      Objective:To observe and analyze the relationship between the spiral CT manifestations and expression of cathepsin D (Cath-D) protein and matrix metalloproteinase -9(MMP-9) in gastric cancer.Methods:This study was performed in gastric cancer tissues of 54 patients with gastric cancer who were treated by surgery in the hospital from September 2015 to September 2014. And selected the gastric mucosa of 54 cases of gastric ulcer patients treated by surgery in hospital at the same period for compared. The expression of MMP-9 and Cath-D protein were detected respectively, and spiral CT diagnosis of all gastric cancer patients.Results:The positive expression rate of MMP-9 and Cath-D protein in gastric cancer tissues were 74.07% and 77.79%, which were significantly higher than 5.56%and 9.26%in normal gastric mucosa, the differences were statistically significant (P<0.05). The expression of MMP-9 and Cath-D were positively correlated(r=0.693, P<0.05). The positive expression rate of MMP-9 and Cath-D protein of lymph node metastasis and of Lauren typing for diffuse type in gastric cancer tissues were significantly higher than those without lymph node metastasis and Lauren typing for intestinal type, the differences were statistically significant (P<0.05). Logistic regression analysis showed that, lymph node metastasis and typing were factors related to the expression of MMP-9 and CT in Cath-D and Lauren expression in gastric cancer tissues.Conclusion:The expression of MMP-9 and Cath-D in gastric cancer tissues was higher than those of normal gastric mucosa, and there is a positive correlation between MMP-9 and Cath-D. The spiral CT manifestations in gastric cancer tissues is correlated with the expression of Cath-D and MMP-9.
