目的:研究同步间歇指令通气(SIMV)与适应性支持通气(ASV)对慢性阻塞性肺部疾病(COPD)合并呼衰患者机体外周血循
环、血气指标的影响。方法:选取2006 年3 月至2013 年12 月在我院接受治疗的COPD 合并呼吸衰竭患者86 例,对其分别先后
采用ASV、SIMV 呼吸模式,记录呼吸机参数、外周血循环参数及血气指标。结果:ASV 组呼吸频率(RR)、潮气量(TV)、吸气峰值
压(PIP)均显著低于SIMV 组(均P<0.01);ASV 组外周血心率(HR)、收缩压(SBP)、舒张压(DBP)、中心静脉压(CVP)均显著低于
SIMV 组(均P<0.01);ASV组动脉血氧分压(PaO2)、pH、血氧饱和度(SaO2)均较SIMV 组显著升高(均P<0.05)。结论:ASV模式
下治疗COPD 合并呼吸衰竭患者的呼吸参数、外周血循环参数、血气改善效果优于SIMV 模式。 |

Objective:To study the influence of synchronous intermittent mandatory ventilation (SIMV) and adaptive support
ventilation (ASV) on the peripheral blood circulation and blood gas indexes in patients with chronic obstructive pulmonary disease
(COPD) and respiratory failure.Methods:A total of 86 patients with COPD combined respiratory failure, who were treated in First
Hospital of Fangshan District from March 2006 to December 2013, were selected and ventilated in ASV and SIMV mode. their The
ventilator parameters, peripheral blood circulation parameters and blood gas indexes were recorded respectively.Results:Respiratory rate
(RR), tidal volume (TV), peak inspiratory pressure (PIP) in ASV group were significantly lower than those in SIMV group(P<0.01).
Peripheral blood heart rate(HR), systolic blood pressure(SBP), diastolic blood pressure(DBP), and central venous pressure(CVP) in ASV
group were significantly lower than those in SIMV group(P<0.01). Arterial partial pressure of oxygen(PaO2), pH, blood oxygen saturation
(SaO2) in ASV group were significantly higher than those in SIMV group (P<0.05).Conclusion:The improvement of ventilator
parameters, peripheral blood circulation parameters and blood gas indexes in ASV mode for patients with COPD combined respiratory
failure was better than in SIMV mode. |
