| 目的：探讨单纯颈椎后路复位CPSS内固定治疗下颈椎骨折脱位伴关节突绞锁的临床效果。方法：选择2010 年1 月～2014
年10 月在我院接受治疗的下颈椎骨折脱位伴关节突绞锁患者95 例，根据手术方法不同分为研究组与对照组。研究组采用单纯
颈椎后路复位CPSS内固定治疗，而对照组采用单纯颈椎前路复位CPSS内固定治疗。观察并比较两组患者的手术效果及神经功
能分级(ASIA)。结果：两组术前ASIA 分级B、C、D 的患者治疗后神经功能恢复较佳，A 级患者治疗后仍为A 级，但较术前明显改
善。两组患者治疗前的ASAI等级在治疗后均有改善，研究组改善程度更明显，差异有统计学意义(P<0.05)。两组均未出现断钉或
螺钉松动等并发症(P>0.05)；两组患者治疗后椎间隙显著增宽，且后路组优于前路组，差异有统计学意义(P<0.05)。与术前比较，两
组术后颈椎后凸畸形Cobb 角均明显改善(P<0.05)，但组间比较差异无统计学意义(P>0.05)。结论：单纯颈椎后路复位CPSS 内固定
治疗下颈椎骨折脱位伴关节突绞锁的临床效果显著，患者神经功能得到明显改善，且术后并发症少，值得推广应用。 |

| Objective:To investigate the clinical effect of posterior cervical lateral reduction and CPSS internal fixation on lower
cervical fractures and dislocations combined with articular process interlocking.Methods:95 patients with lower cervical spine with articular
line lock who were treated in our hospital from January 2012 to October 2014 were selected as the research objects. 65 cases accepted
pure simple cervical posterior reduction and internal fixation CPSS were considered as the research group, and another 30 cases
accepted anterior reduction and internal fixation CPSS were considered as the control group. Then the clinical effect was observed and
compared and the nerve function recovery was evaluated by the American Spinal Injury Association (ASIA).Results:The nerve functions
of patients in two groups who were divided into B, C and D grade before treatment all recovered well, patients at A grade with no
progress, but was better than before. The ASAI of patients in two groups after treatment improved when compared with before, and the
research group improved was better than that of the control group (P<0.05). No statistical significance was found in the postoperative
complications such as broken nail or screw loose between two groups (P>0.05). After the treatment, the intervertebral disc in the two
groups significantly widened, and the research group was higher than that of the control group (P<0.05). The cervical kyphosis Cobb angles
were significantly improved in the two groups after treatment (P<0.05), but no statistical significance was observed between two
groups (P>0.05).Conclusion:Cervical spine fracture dislocation with articular process under the ground lock line simple cervical posterior
reduction and internal fixation CPSS's had good effect. |
