| 目的：探讨老年维持性血液透析(MHD)患者血清脂联素(adiponectin，ADPN)水平与其心脑血管事件发生风险及其预后的关
系。方法：采用酶联免疫吸附实验(ELISA)检测76 例老年MHD 患者血清ADPN 水平，以5 mg/L为界，以<5 mg/L为低ADPN组，
≥ 5 mg/L为高ADPN 组。随访观察两组心脑血管事件的发生情况及预后。采用Cox 回归分析血清ADPN水平和心脑血管事件对
老年MHD 患者的预后影响。结果：76 例老年MHD 患者的血清ADPN水平为(11.10± 10.68) mg/L，其中低ADPN 组患者有33
例，高ADPN 组患者有43 例。与低ADPN 组相比，高ADPN 组患者的心脑血管事件发生率明显下降，而生存时间明显延长(P<0.
05)。Cox 回归分析显示低ADPN 水平和发生心脑血管事件是老年MHD 患者生存时间的危险因素(P<0.05)。结论：血清ADPN 水
平可作为老年MHD 患者心脑血管事件的预测指标，并与患者的预后相关，有较好的临床应用价值。 |

| Objective:To explore correlation of serumadiponectin (ADPN) level and cardiac and cerebral events and prognosis in
elderly patients with maintenance hemodialysis (MHD).Methods:Serum ADPN levels were detected by ELISA method in 76 elderly
MHD patients, and the patients divided into low ADPN group (<5 mg/L) and high ADPN group (≥ 5 mg/L) by serum ADPN level. The
cardiac and cerebral events and prognosis of patients were observed follow-up between groups, and the effect of serum ADPN level and
cardiac and cerebral events on prognosis of elderly MHD patients was analyzed by Cox regression analysis.Results:Serum ADPN level
of elderly MHD patients was (11.10± 10.68) mg/L, and there were 33 cases in low ADPN group and 43 cases in high ADPN group.
Compared to low ADPN group, the incidence of cardiac and cerebral events decreased and survival time lengthened in patients of high
ADPN group (P<0.05). Cox regression analysis showed low ADPN level and cardiac and cerebral events were risk factors of elderly
MHD patients (P<0.05).Conclusion:ADPN level can be used as a predictor for cardiac and cerebral events, and close to prognosis in
elderly MHD patients. It has better clinical application value. |
