      目的: 探讨成纤维细胞生长因 子诱导 14(Fn1 4)在表皮生长因子受体(EGFR)外显子 19 缺失的非小细胞肺癌(NSCLC)组织中 的表达及其临床意义。 方法: 选择 2010 年 9 月 至 2013 年 11 月 第四军医大学唐都医院收治的 1 25 例 原发性 EGFR 外显子 19 缺 失的 NSCLC 组织及与 之相对应的 30 例 正常肺组织为研究对象, 应用 免疫组化法检测和比较其 Fn14 的表达, 分析 Fn14 的表达 与 原发性 EGFR 外显子 19 缺失的的 NSCLC 临床病理特征的关系。结果: Fn14 阳性表达主要定位于胞膜和胞质中。在 125 例原发 性 EGFR 外显子 19 缺失的 NSCLC 组织中, Fn14 的阳性表达率为 100%,显著高于正常肺组织(P<0.001)。 鳞癌和腺癌 NSCLC 组 织中 Fn14 的阳性表达率比较无统计学差异(P=0.1 06); 不同 病理分化程度、不同 TNM 分期的 NSCLC 组织中 Fn14 的阳性表达率 比较均具有显著性差异(P=0.000、P=0.000)。 Fn14 蛋白的表达与 EGFR 外显子 19 缺失的 NSCLC 的淋巴结转移状态、肿瘤大小以 及肿瘤解剖学分类均显著相关(P=0.000、 P=0.029、 P=0.000、P=0.026),而与 患者的性别、年龄、吸烟史均无关(P=0.816、 P=0.122、P=0. 816)。 结论: Fn14 在原发性 EGFR 外显子 19 缺失的 NSCLC 中呈异常高表达, EGFR 外显子 19 缺失突变很可能通过上调 Fn14 通 路, 促进 NSCLC 的发生和发展。

      Objective:To investigate the expression of fibroblast growth factor-inducible 14 (Fn14) in NSCLC tissues with deletion of EGFR exon 1 9 and its correlation with clinicopathological features.Methods:1 25 NSCLC tissues with primary deletion of EGFR exon 19 and 30 corresponding normal lung samples were chosen between September 201 0 and November 201 3 in TangduHospital, Fourth Military Medical University, and Fn1 4 was detected in these samples and the relationships of Fn1 4 expressional rate in these cancer samples with the pathological grade and clinical stage were analyzed.Results:The expression of Fn14 was mainly located in cell membrane and cytoplasm. In 125 NSCLC EGFR1 9 deletion samples, the expression rate of Fn1 4 protein is 1 00%, which showed extremely significance compared with the corresponding normal lung tissue (P < 0.001 ). There was no statistical difference of Fn1 4 positive expression between the squamous carcinoma group and the adenocarcinoma group (P = 0.106); while there were very significant difference of Fn14 positive expression in the different pathological grade and TNM stage of NSCLC tissues (P=0.000, P=0. 000). There also was significant correlation of the Fn1 4 protein expression with TNM stages, lymph node metastasis status, tumor size, and tumor location groups of NSCLC with EGFR 19 deletion (P=0.000, P=0.029, P=0.000, P=0.026). However, there was no significant correlation of Fn1 4 expression with the gender, age and smoking history groups (P=0.816, P=0.122, P=0.816).Conclusion:The Fn14 proteins are highly expressed in NSCLC tissues with primary deletion of EGFR exon 1 9, and EGFR exon 1 9 deletion mutation might be the causing reason raising Fn1 4 pathway to promote occurrence and development of NSCLC
