      目的:探讨不同氨溴索给药方式对大鼠机械通气肺部炎性因子释放及肺组织湿/ 干比值的影响。方法:将45 只健康SD 雄性 大鼠随机分成静脉组、雾化组和对照组,各15 只。对照组:大鼠麻醉后行气管切开插管,并行单纯机械通气4h;静脉组:大鼠麻醉 后从尾静脉泵入氨溴索,气管切开插管后行机械通气4h;雾化组:大鼠麻醉后行气管切开插管,通过自制装置氧气驱动雾化吸入 氨溴索,并行机械通气4 h。各组机械通气后放血处死大鼠,取肺组织,计算肺湿/干重比(W/D)。收集支气管灌洗液(BALF)并检测TNF-alpha,IL-1beta和IL-6 浓度。结果:与对照组比较,静脉组和雾化组大鼠支气管灌洗液中TNF-alpha,IL-1beta和IL-6 的浓度明显降低 (P<0.05),但此两组间无显著性差异(P>0.05);与对照组比较,静脉组和雾化组大鼠肺组织W/D 比也明显降低(P<0.05),但雾化组 大鼠肺组织W/D 比明显低于静脉组大鼠(P<0.05)。结论:氨溴索静脉给予和雾化给予都能显著降低大鼠机械通气时肺内炎性因 子水平和肺W/D 比,而且雾化给予较静脉给予更能减轻大鼠肺组织的W/D 比。

      Objective:To investigate the effect of ambroxol with different administration routes on inflammatory factor and wet/dry ratio of lung tissues of rats undergoing ventilation.Methods:45 male rats were randomly assigned into three groups, control group (n=15), intravenous injection (IV) group (n=15) and inhalation group (n=15). In control group, the rats received tracheostomy and ventilation for 4h; in intravenous injection group, the rats received tracheostomy and ventilation for 4h, meanwhile ambroxol was pumped into caudal vein of rats; in inhalation group, the rats received tracheostomy and ventilation for 4 h, meanwhile we used a homemade device to oxygen ambroxol into rats’lung. After 4 hours' ventilation, we executed all rats and obtained its lung tissue and brocho-alveolar lavage fluid (BALF) to calculate the wet/dry ration of lung tissues and test the levels of TNF-alpha, IL-1beta and IL-6 in BALF.Results:Compared with control group, the levels of TNF-alpha, IL-1beta and IL-6 significantly decreased in IV group and inhalation group (P<0.05), but there was no significant difference between IV group and inhalation group (P>0.05). Compared with control group, the wet/dry ratio in lung tissue was significantly decreased in both IV group and inhalation group (P>0.05). And the ratio in inhalation group was lower than in IV group (P>0.05).Conclusion:Both intravenous injection and inhalation of ambroxol could decrease the levels of inflammatory factors and wet/dry ratio of lung tissue in rats undergoing mechanical ventilation. And compared with intravenous injection, inhalation of ambroxol could reduce the wet/dry ratio of lung tissue more efficiently.
