      目的:探讨内镜下逆行胰胆管造影术(ERCP)下塑料胆道支架引流术治疗复杂性胆总管结石的临床疗效和安全性。方法:回 顾性分析2011 年9 月至2013 年9 月在我院经ERCP 下胆道支架引流术治疗的32 例复杂性胆总管结石患者的临床病例资料。 结果:32 例患者塑料胆道支架引流术全部成功,平均手术时间15-30 分钟。术后,2 例发生高淀粉酶血症,经禁食72小时后恢复正 常,无穿孔、消化道大出血等ERCP 严重并发症发生。术后1 周,患者腹痛、发热消失,转氨酶及胆红素水平明显下降,平均住院时 间6-15 天。3 个月复查B 超,发现结石缩小19 例,结石碎裂1 例,支架脱落1 例。术后7 天、术后3 个月的肝功能指标与术前比较 均显著改善,差异均有统计学意义(P<0.05)。结论:ERCP下塑料胆道支架引流术是一种复杂性胆总管结石安全有效的治疗方法, 具有创伤小、风险较低、操作时间短、患者易耐受及手术成功率高等优点。
英文摘要:
      Objective:To explore the clinical efficacy and safety of endoscopic retrograde cholangiopancreatography(ERCP) with plastic stents in the treatment of patients with complex biliary stones.Methods:The clinical data of 32 patients who had undergone ERCP for complex biliary stones in our hospital from September 2011 to September 2013 were retrospectively reviewed.Results:All 32 cases were successfully treated by ERCP. 8 patients had huge common bile duct stones. 9 had bile duct multiple surgeries after cholecystectomy and combined obstructive suppurative cholangitis, respectively. 2 had diverticulum nipple and cirrhosis, respectively. 3 had gallstone pancreatitis and 1 had diverticulum next to the nipple, pancreatic duct stent implantation and liver abscess, respectively.Conclusion:For patients with complex biliary stones, ERCP with plastic stents was a kind of safe and effective treatment which had advantages such as small trauma, low risk rate, short operation time, better patient tolerance and high success rates.
