      目的:研究人工髋关节置换术中股骨假体周围骨折的影响因素及其相关性,为人工髋关节置换术中股骨假体周围骨折的预 防和诊治提供科学依据。方法:选取本院2012 年11 月至2013 年9 月施行髋关节置换手术的患者780 例, 搜集各研究对象的一 般资料(年龄、性别等)以及随访观察其髋关节置换手术后股骨假体周围骨折的发生情况,把股骨假体周围骨折的发生情况与性 别、年龄、固定类型等因素进行Logistic 回归分析。结果:780 例施行髋关节置换手术的患者中,有52 例患者术后出现股骨假体周 围骨折,总发生率为6.7%;其中A型31 例,B 型18 例(B1 型6 例,B2 型7 例,B3 型5 例),C 型3 例。女性的发生率(8.5%)高于 男性(4.7%),老年患者中股骨假体周围骨折的发生率最高(10.1%),生物型假体固定者的发生率(8.7%)高于骨水泥型假体固定者 (3.8%),人工髋关节翻修术中股骨假体周围骨折的发生率最高(11.4%),股骨假体周围骨折发生的危险因素为女性、高龄、使用生 物型假体固定,OR 值分别为5.301、3.675 和2.336。结论:人工髋关节置换术中的危险因素为女性、高龄、使用生物型假体固定,临 床医生应在术前对行人工髋关节置换术手术者进行充分的评估,制定合理的手术方案及选择合适的固定方式。
英文摘要:
      Objective:To analyse the influence factors of periprosthetic femoral fractures post hip arthroplasty and provide scientific basis for periprosthetic femoral fractures' diagnosis and treatment.Methods:780 patients who were treated in our hospital from November 2009 to September 2013 were selected, patient's general data (age, gender, etc.)were collected and occurrence of periprosthetic femoral fractures were observed.Periprosthetic femoral fractures' situation and age, gender, fixing type were analysed via logistic regression analysis.Results:In this 780 hip arthroplasty surgery patients, there are 52 patients who undergo periprosthetic femoral fractures after surgery and the total rate is 6.7%; among them, 31 patients is A type, 18 patients is B type, 3 patients is C type; the female's occurrence rate(8.5%)is higher than male's(4.7%); the occurrence rate of old patients(10.1%)is the highest; the occurrence rate of patients who used biological prosthesis fixing (8.7%)is higher than the occurrence rate of patients who used bone cement prosthesis fixing(3.8%); the occurrence rate of patients who need a artificial hip revision procedures is the highest(11.4%); the risk factors of periprosthetic femoral fractures during hip arthroplasty is female, old age and use biological prosthesis fixed, the OR value is 5.301, 3.675 and 2.336 respectively.Conclusion:The risk factors of periprosthetic femoral fractures post hip arthroplasty is female, old age and application of biological prosthesis fixing, clinical doctors need to make a adequate assessment for patients before hip replacement surgery ,formulate reasonable operation scheme and choose the right fixing means.
