| 摘要目的：探讨筋膜下经内镜穿通静脉离断术(SEPS)治疗下肢静脉性溃疡的远期临床效果。方法：收集1999 年12 月至2001 年
1 月在本院接受SEPS治疗的36 例患者，共40 条患肢，均患有静脉性溃疡。所有患者同时行大隐静脉高位结扎及曲张浅静脉剥
脱术，术后1、3、6、12 个月接受随访，以后每年随访1 次，随访时间10 年，观察患者的近期和远期疗效。结果：所有患肢溃疡均愈
合，2 例复发。1 条患肢出现神经损伤，2 条患肢出现皮下血肿。16 条患肢出现丹毒，其中5 条患肢反复发作，6 条患肢静脉曲张复
发。结论：SEPS 治疗下肢静脉性溃疡的一种有效方法，近期并发症少，远期并发症率高。在促进溃疡愈合方面与传统手术相比未
见明显优势。 |

| ABSTRACT Objective:To investigate the long-term therapeutic effect of patients with lower limb venous ulcers treated by SEPS.
Methods：SEPS was performed on 40 limbs in 36 patients with venous ulcers while high ligation with stripping combined with varicosity
avulsion was also performed in every affected limb. All the patients were followed up in 1,3,6,12 months and every year after SEPS during
ten years. Results：ALL ulcers healed but ulcer recurrence was presented in 2 limbs. Nerve injury, hematoma and varicose veins
recurrence were presented in 1, 2 and 6 limbs, respectively. Erysipelas was presented in 16 limbs which was recurred in 5 limbs
repeatedly. Conclusion:SEPS was an effective procedure for low limb venous ulcers. Complication ratio was low in short-termresult and
high in long-termresult. The effect of SEPS for promoting ulcers healing was not significantly different with that of open surgery alone. |
