      目的:研究绝经后女性高尿酸血症患者血浆中性激素结合球蛋白(sex hormone binding globulin ,SHBG)水平与血尿酸水平 的相关性。方法:选取绝经后女性404 例,其中高尿酸血症组204 例,正常对照组200 例,测量所有研究对象体重、身高、腰围、臀 围、收缩压(SBP)、舒张压(DBP),并计算体重指数(BMI)和腰臀比(WHR),检测血尿酸(UA)、空腹血糖(FBG)、总胆固醇(TG)、甘 油三酯(TC)低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、雌激素(E2)、雄激素(T)、空腹胰岛素(INS)及SHBG,并计算胰岛素抵抗 指数(Homa-IR)。结果:与绝经后女性尿酸正常者相比,高尿酸血症的患者UA, WHR, TC,FBG, INS, Homa-IR明显升高(P<0.01), DBP,WAIST 和LDL升高( P<0.05),SHBG 水平明显下降(P<0.01);SHBG与INS、UA 、TG呈显著负相关(P<0.05),SHBG与E2 呈 显著正相关P<0.01)。结论:绝经后女性中高尿酸血症患者的低血浆SHBG水平与高血尿酸水平显著相关,血浆SHBG水平下降 与胰岛素抵抗可能高度相关,低SHBG 可能作为绝经后女性患高尿酸血症的高危因素。

      Objective:The aim of this study was to determine sex hormone binding globulin (SHBG) concentration in postmenopausal women with hyperuricemia and to evaluate the relationships between SHBG and serum uric acid.Methods:400 cases of postmenopausal women, including 200 hyperuricemia cases and normal control group of 200 cases, were included in this study. All subjects were measured for weight, height, waist circumference, hip circumference, systolic blood pressure (SBP), diastolic blood pressure (DBP), and body mass index (BMI) and waist-hip ratio (WHR), detection of serum uric acid (UA), fasting glucose (FBG), total cholesterol (TG), triglycerides (TC), low-density lipoprotein (LDL), high density lipoprotein (HDL), estrogen (E2), testosterone(T), fasting insulin (INS) and SHBG, and insulin resistance index (Homa-IR). Results:Compared with women in control group, in patients with hyperuricemia UA, WHR, TC, FBG, INS, Homa-IR in patients with hyperuricemia were significantly higher (P<0.01), and DBP, WAIST and LDL were also higher (P<0.05);while SHBGsignificantly decreased (P<0.01). SHBGwas significantlynegatively correlated with INS,UAand TG (P<0.05); SHBG and E2 has a significant positive correlation (P<0.01).Conclusion:The plasma SHBG levels in postmenopausal women with hyperuricemia are lower and may be associated with insulin resistance and abdominal obesity. Low plasma SHBG level is a hyperuricemia risk factor for postmenopausal women.
