| 目的：探讨免疫固定电泳(immunofixation electrophoresis，IFE)检测对浆细胞肿瘤相关肾损害的诊断价值。方法：回顾性分析
我院肾病科665 例住院患者的免疫固定电泳分型结果及检出M蛋白带的患者骨髓检查结果。结果：665 例肾病科患者中检出44
例单克隆免疫球蛋白带，检出率6.6%，其中IgG型24 例(κ型6 例，λ型18 例)，IgA 型10 例(κ型6 例，λ型4 例)，IgM型2 例(均
为κ型)，单纯轻链κ型2 例，单纯轻链λ型6 例；此44 例患者经骨髓检查，38 例确诊为多发性骨髓瘤(multiple myeloma，MM)，2
例确诊为巨球蛋白血症(waldenstrom macroglobulinemia，WM)，骨髓涂片中骨髓瘤细胞比例均≥ 10%；骨髓瘤细胞＜10%的4 例，
诊断为意义未明单克隆免疫球蛋白血症(monoclonal gammopathy of un-determined significance，MGUS)。结论：免疫固定电泳检测
可对浆细胞肿瘤相关肾损害提供重要的诊断和分型依据，值得临床应用。 |

| Objective:To investigate the Clinical value of immunofixation electrophoresis on diagnosis of plasmacytoma related
renal impairment.Methods:665 cases IFE records of patients with department of nephropathy in our hospital were retrospectively
analyzed, further analyzed typingM proteins and myeloma cell ratio in bone marrow.Results:665 cases of patients with department of
nephropathy detection in 44 cases of monoclonal immunoglobulin, the detection rate 6.6%, among which IgG type 24 cases (κtype 6
cases, λtype 18cases), IgA type 10 cases (κtype 6 cases, λtype 4 cases), type 2 cases of IgM(both for κtype), light chain κtype 2 cases,
light chain λtype 6; The 44 patients after bone marrow examination, and 38 cases were diagnosed as MM, 2 cases were diagnosed as
WM, myeloma cell percentage ≥ 10 %;4 cases were diagnosed as MGUS, myeloma cell percentage < 10%.Conclusion:Immunofixation
electrophoresis detection of plasmacytoma related renal impairment to provide important basis in diagnosis and classification, worthy of
clinical application. |
