      目的:探讨尿视黄醇结合蛋白(RBP)、尿N- 乙酰-β-D- 氨基葡萄糖苷酶(NAG)、尿胱蛋白酶抑制剂C(Cystatin C)的测定对 原发性高血压早期肾损害诊断的敏感性及临床意义。方法:选择2011 年4 月~2011 年11 月在新疆自治区人民医院肾病科住院 的原发性高血压患者69 例作为观察组,另设健康体检者30 例为对照组。尿RBP、Cystatin C 测定采用酶联免疫法(ELISA)检测, 尿NAG 测定采用对硝基苯酚(PNP)比色法检测。结果:观察组中尿RBP、NAG、Cystatin C 的测定含量明显高于正常对照组(P< 0.001),通过ROC 曲线、诊断试验结果显示:尿RBP 的曲线下面积为0.962,95%可信区间为0.921~0.983,较尿NAG、Cystatin C 更具有敏感性(P<0.001)。结论:检测尿RBP 有助于高血压肾损害的早期监测,对预防高血压性肾病的发生、发展具有重要的临 床价值。
英文摘要:
      Objective: The sensitive index and clinical significance in early renal injury caused by primary hypertension was investigate by detecting Urine retinol combined with protein(RBP), N-acetyl-β-D-amino grape glycosidase (NAG), urine micro proteinase inhibitor C (Cystatin C). Methods: 69 patients with primary hypertension were enrolled who were in-patients, dated from April 2011 to November 2011; 30 subjects of healthy people for the outpatient were selected as control group. The urine RBP and urine Cystatin C were respectively determined by enzyme linked immunosorbent assay (ELISA). Urine NAG was measured by p-nitrophenol (PNP) colorimetric assay. Results: In case group urine RBP, NAG and the Cystatin C determination content were higher that in the normal control group, the differences were statistically significant (P<0.001).Through the ROC curve and diagnostic test, the area under the curve of Urine RBP were 0.962, the 95% confidence interval were 0.921~0.983(P<0.001). Conclusions: The urine RBP could be helpful in early detection of hypertensive renel injury, and there was important clinical value in the prevention of hypertensive renel injury.
