      目的:探讨瘦素受体基因Lys109Arg 多态性与慢性阻塞性肺疾病营养状况的关系。对象与方法:观察159 例COPD 稳定期患 者及110 例健康对照者体重指数(BMI)、理想体重百分比(NW%)、三头肌皮皱厚度(TSF)、上臂中点臂围(MAC)、血清白蛋白 (ALB)、总淋巴细胞(LYM)等营养参数,将COPD 组分为营养不良组(COPD1 组)68 例,COPD 非营养不良组(COPD2 组)91 例。用 酶联免疫吸附试验(ELISA) 法测定血清瘦素水平,采用聚合酶链式反应及连接酶检测反应方法(PCR -LDR) 测定瘦素受体 Lysl09Arg 多态性的基因型。结果:COPD1 组Lys109Arg 基因型GG、GA 及AA 的频率分别为0.838、0.147 和0.015,G 和A 等位 基因分别为0.912 和0.088;COPD2 组Lys109Arg 基因型GG、GA 及AA 的频率分别为0.67、0.319 和0.011,G 和A 等位基因分 别为0.83 和0.17;对照组Lys109Arg 基因型GG、GA 及AA 的频率分别为0.7、0.273 和0.027,G 和A 等位基因分别0.841 和 0.159;COPD1 组Lys109Arg 基因型及等位基因频率与COPD2 组和对照组比较差异有显著性;COPD2 组和对照组比较差异无显 著性。GG 型受试者血清瘦素水平低于A/G 型+AA 型(39.08±15.79ng/ml vs 43.29±17.25ng/ml),但差异无统计学意义。结论:瘦 素受体基因Lys109Arg 多态性可能与COPD 营养状况相关。

      Objective: To investigate the association between Leptin Receptor Gene polymorphism Lys109Arg and nutriture of Patients with Chronic Obstructive Pulmonary Disease. Methods: A hundred and fifty-nine COPD patients in stable stage and a hundred and ten normal controls were studied. Nutritional parameters, including body mass index (BMI), percentage of normal body mass (NM%), triceps skin-fold thickness(TSF), mid-upper arm circumference(MAC), serum album(ALB), total lymphocyte counts(LYM) were determined. COPD patients were divided into malnutrition group (group 1) and non-malnutrition group (group 2) according to the nutrition parameter. Serum leptin levels were measured by ELISA. The frequencies of genotype and allele single nucleotide polymorphism of leptin receptor gene (Lys109Arg) were detected and compared with polymerase chain reaction-ligase detection reaction. Results: In patients of COPD with malnutrition, the frequencies of genotype GG, GA, and AA were 0.838, 0.147 and 0.015 respectively, while those in patients of COPD without malnutrition were 0.67, 0.319 and 0.011 respectively, while those in controls were 0.7, 0.273 and 0.027; the allele frequencies of G and A in patients of COPD with malnutrition were 0.912 and 0.088, while those in patients of COPD without malnutrition were 0.83 and 0.17, while those in controls were 0.841 and 0.159. Significant differences were detected in genotypes and allele frequencies of Lys109Arg between COPD1 subjects, COPD2 subjects and controls, but there was no obvious difference between COPD2 subjects and controls. And there were no significant difference in leptin levels between GG, A/G and AA genotype. Conclusion: Results suggests that LEPR Lys109Arg polymorphism is possibly associated with nutriture of Patients with Chronic Obstructive Pulmonary Disease.
