      目的:探讨尿动力学检查在了解慢性非细菌性前列腺炎/ 慢性骨盆疼痛综合征(Chronic abacterial prostatitis/chronic pelvic pain syndrome , CPPS)患者中下尿路症状(LUTS)产生原因的作用。方法:对36例难治性慢性前列腺炎/盆腔疼痛综合征患者行尿 流动力学压力-流率测定,同步测定膀胱压、逼尿肌压、同步肌电图测定,了解其症状产生的原因。结果:36例患者中,尿动力学证 实膀胱出口梗阻14 例( 39% );逼尿肌过度活动者8 例,其中有7 例与BOO 同时存在;假性逼尿肌尿道外括约肌协同失调6 例 (16.7% );逼尿肌收缩力低下者5例(13.9%)。结论:对难治性CPPS患者进行尿动力学检查有助于对此类患者LUTS产生的原因 进行鉴别,从而可以采取有针对性的治疗。

      Objective: Investigate the urodynamic findings in chronic abacterial prostatitis/chronic pelvic pain syndrome (CPPS) and discuss the causes of the lower urinary symptoms coexistence with the patients of CPPS. Methods: 36 cases of CPPS underwent clinical evaluation from May 2008 to March 2011, including NIH-CPSI、International Prostate Symptom Score (IPSS), Peak Flow rate (PF) and Residual Urine (RU) measurement. Pressure flow studies were performed. Results: Mean patient age was 33 years (range 20 to 50). The NIH-CPSI and IPSS were 23.6 ± 3.3 and 13.6 ± 5.5 respectly; Bladder outlet obstruction, detrusor overactivity, pseudodyssynergia and detrusor underactivity were the main urodynamic results of these patients. Most of these patients showed a decrease of the PF, mean first sensation of bladder and cystometric capacity. Conclusions: To the patients of CPPS, the coexistence of voiding dysfunction may lead to the longer duration of symptoms. Urodynamic studies provide us a better understanding of the nature of voiding dysfunction in these patients and enable appropriate treatment to improve symptoms. This study emphasis the importance of the urodynamic studies in the the patients with CPPS.
