      摘要 目的:分析不同幕下区肿瘤与脑干听觉诱发电位(BAEP)指标的相关性,为幕下区肿瘤的临床鉴别和术前诊断评估提供参考。方法:回顾性分析136例幕下区脑肿瘤患者的临床资料。术前对幕下区脑肿瘤患者行BAEP指标检测,包括各波潜伏期(PL)、峰间期(IPL)、侧间峰潜伏期差值(ILD)。比较幕下区各肿瘤组PL、IPL、ILD、各波异常率、各组不同肿瘤直径BAEP指标异常率。结果:VS组77例(95.06%)双侧BAEP检测异常;脑膜瘤组25例双侧(75.76%)BAEP检测异常;混合组18例(81.82%)双侧BAEP检测异常。VS组Ⅰ、Ⅲ、Ⅴ波的PL长于脑膜瘤组,且脑膜瘤组Ⅰ、Ⅲ、Ⅴ波的PL长于混合组(P<0.05)。VS组患侧Ⅴ波ILD长于脑膜瘤组,且脑膜瘤组的患侧Ⅴ波ILD长于混合组(P<0.05)。VS组V波的PL异常率高于脑膜瘤组,且脑膜瘤组高于混合组(P<0.05)。VS组I-V波的IPL异常率高于脑膜瘤组,且混合组高于脑膜瘤组(P<0.05)。不同肿瘤直径V波的ILD组间比较具有统计学差异(P<0.05),且随着肿瘤直径增大,V波的ILD的越大(P<0.05)。结论:BAEP指标其Ⅰ波、Ⅲ波和Ⅴ波PL对幕下区肿瘤的分型鉴别具有一定参考意义,尤其是Ⅴ波ILD值与肿瘤直径和类型均有一定的相关性。

      ABSTRACT Objective: To analyze the correlation between different infratentorial tumors and brainstem auditory evoked potential (BAEP) indexes, and to provide reference for clinical identification and preoperative diagnosis and evaluation of infratentorial tumors. Methods: The clinical data of 136 patients with infratentorial brain tumors were retrospectively analyzed. BAEP indexes were detected in patients with brain tumors in the infratentorial area before operation, and related neurophysiological indexes were recorded, including latency (PL), interpeak latency (IPL) and interpeak latency difference (ILD). The PL, IPL, ILD, abnormal rate of each wave, abnormal rate of BAEP index of different tumor diameters in different group were compared. Results: 77 cases (95.06%) had bilateral BAEP abnormalities in VS group. 25 cases (75.76%) had abnormal BAEP detection on both sides in meningioma group. 18 cases (81.82%) had abnormal bilateral BAEP detection in mixed group. The PL of I, III and V waves in VS group was longer than that in meningioma group, and the PL of I, III and V waves in meningioma group was longer than that in mixed group(P<0.05). V-wave ILD of the affected side in VS group was longer than that in meningioma group, and the V-wave ILD of the affected side in meningioma group was longer than that in mixed group(P<0.05). The abnormal rate of PL of V wave in VS group was higher than that in meningioma group, and meningioma group was higher than that of mixed group (P<0.05). The abnormal rate of IPL of I-V wave in VS group was higher than that in meningioma group, and mixed group was higher than that of meningioma group(P<0.05). The ILD of V wave in different tumor diameters was statistically different (P<0.05), and the ILD of V wave increased with the increase of tumor diameter (P<0.05). Conclusion: The I wave, III wave and V wave PL of BAEP index have certain reference significance for the classification and identification of infratentorial tumors, especially the V wave ILD value has a certain correlation with tumor diameter and type.
