摘要 目的:探讨磁共振单次激发快速自旋回波(SSFSE)序列联合扩散加权成像(DWI)在产前胎儿头颈部占位性病变定位定性诊断中的应用价值。方法:选取2018年10月~2023年10月来本院检查的胎儿,回顾性分析66例经超声筛查怀疑头颈部占位性病变的胎儿磁共振SSFSE及DWI数据信息,观察病变部位、信号特征以及胎儿头颈部占位性病变定位、定性的诊断与超声诊断一致性做对比。结果:SSFSE序列联合DWI序列清晰准确地显示出胎儿头颈部占位性病变的部位及性质,66例病例中囊肿约占57.6%(38/66)、血肿30.3%(20/66)、肿瘤12.1%(8/66);其中囊肿常见的是蛛网膜囊肿、室管膜下囊肿,血肿发生的位置多以脑室内血肿居多;对于胎儿头颈部占位性病变产前磁共振SSFSE序列联合DWI序列诊断准确性(66/66,100%)优于超声诊断准确性(53/66,80%)(P<0.001)。结论:SSFSE联合DWI序列能准确定位和定性胎儿头颈部占位性病变,在临床诊断、指导治疗中具有较高的应用价值。 |

ABSTRACT Objective: To explore the clinical application of magnetic resonance single-shot fast spin-echo (SSFSE) sequence combined with diffusion-weighted imaging (DWI) in the localization and qualitative diagnosis of prenatal fetal head and neck space-occupying lesions. Methods: The magnetic resonance SSFSE and DWI data of 66 fetuses with suspected head and neck space-occupying lesions after ultrasound screening from October 2018 to October 2023 were retrospectively analyzed. Compare the consistency between qualitative diagnosis and ultrasound diagnosis by observing the location of lesions, signal characteristics, and the localization of fetal head and neck space-occupying lesions. Results: The combination of SSFSE sequence and DWI sequence clearly and accurately displays the location and nature of fetal head and neck space-occupying lesions. Among the 66 cases, cysts accounted for approximately 57.6% (38/66), hematomas accounted for 30.3% (20/66), and tumors accounted for 12.1% (8/66). The common cysts were arachnoid cysts and subventricular cysts. The location of the hematoma was mostly intraventricular hematoma. For the diagnosis of fetal head and neck space-occupying lesions, the diagnostic accuracy of prenatal magnetic resonance SSFSE sequence combined with DWI sequence (66/66, 100%) was higher than that of ultrasound diagnosis (53/66, 80%)(P<0.001). Conclusion: The combination of SSFSE and DWI sequence can accurately locate and characterize fetal head and neck space-occupying lesions and has a high application value in clinical diagnosis and treatment guidance. |
