      目的:探讨推拿罐结合刺络放血疗法对椎动脉型颈椎病(气滞血瘀型)的治疗效果。方法:选取2021年4月~2023年11月期间我院收治的84例椎动脉型颈椎病(气滞血瘀型)患者,采用随机数字表法将其分为对照组(n=42,甲钴胺片+常规针刺治疗)和研究组(n=42,甲钴胺片+推拿罐结合刺络放血疗法治疗)。对比两组视觉模拟评分法(VAS)评分、颈性眩晕症状与功能评估量表(ESCV)评分、椎动脉型颈椎病功能评定量表(FS-CSA)评分、炎症介质水平[白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)]、血管内皮功能指标水平[内皮素-1(ET1)、一氧化氮(NO)]、椎动脉血流速度指标水平[收缩期血流速度(Vs)、舒张期血流速度(Vp)、血管搏动指数(PI)、血管阻力指数(RI)]。结果:治疗后,两组VAS评分、ESCV评分、FS-CSA评分降低,且研究组降低幅度大于对照组(P<0.05)。治疗后,两组血清IL-6、TNF-α、ET-1水平降低,且研究组降低幅度大于对照组;血清NO水平升高,且研究组升高幅度大于对照组(P<0.05)。治疗后,两组PI、RI降低,且研究组降低幅度大于对照组;Vs、Vp升高,且研究组升高幅度大于对照组(P<0.05)。结论:推拿罐结合刺络放血疗法治疗椎动脉型颈椎病(气滞血瘀型),可有效改善患者椎动脉血流速度和眩晕症状,促进颈椎功能恢复,减轻机体疼痛程度,可能与调节炎症因子水平和血管内皮功能有关。

      Objective: To explore the therapeutic effect of tuina cupping combined with pricking bloodletting therapy on cervical spondylosis of vertebral artery type (qi stagnation and blood stasis type). Methods: A total of 84 patients with cervical spondylosis of vertebral artery type (qi stagnation and blood stasis type) admitted to our hospital from April 2021 to November 2023 were selected and divided into control group (n=42, mecobalamin tablets+conventional acupuncture treatment) and study group (n=42, mecobalamin tablets+tuina cupping combined with pricking bloodletting therapy) by random number table method. The visual analogue scale (VAS) score, cervical vertigo symptom and function assessment scale (ESCV) score, vertebral artery type cervical spondylosis function assessment scale (FS-CSA) score, inflammatory mediator level [interleukin (IL)-6, tumor necrosis factor-α (TNF-α)], vascular endothelial function index level [endothelin-1 (ET1), nitric oxide (NO)], vertebral artery blood flow velocity index level [systolic blood flow velocity (Vs), diastolic blood flow velocity (Vp), vascular pulsation index (PI), vascular resistance index (RI)] were compared in two groups. Results:After treatment, the VAS score, ESCV score and FS-CSA score of the two groups decreased, and the decrease of the study group was greater than that of the control group (P<0.05). After treatment, the levels of serum IL-6, TNF-α and ET-1 in the two groups decreased, and the decrease in the study group was greater than that in the control group. Serum NO level increased, and the increase in the study group was greater than that in the control group (P<0.05). After treatment, the PI and RI of the two groups decreased, and the decrease of the study group was greater than that of the control group. Vs and Vp increased, and the increase in the study group was greater than that in the control group (P<0.05). Conclusion: Tuina cupping combined with pricking bloodletting therapy in the treatment of cervical spondylosis of vertebral artery type (qi stagnation and blood stasis type), can effectively improve the vertebral artery blood flow velocity and vertigo symptoms, promote the recovery of cervical spine function, and reduce the degree of pain in the body, which may be related to the regulation of inflammatory factor levels and vascular endothelial function.
