      目的 探讨口腔扁平苔藓(OLP)患者外周血单个核细胞肿瘤坏死因子样配体1A(TL1A)、可溶性程序性死亡受体1(sPD-1)信使核糖核酸(mRNA)表达与辅助性T细胞17(Th17)/调节性T细胞(Treg)失衡的相关性分析。方法 选择2020年1月~2023年12月在我院治疗的OLP患者133例(OLP组),根据患者损害类型分为糜烂型组(74例)和非糜烂型组(59例),另选择同时间段我院113例体检健康者作为对照组。检测外周血单个核细胞TL1A mRNA、sPD-1 mRNA表达和外周血维甲酸相关孤核受体γt(RORγt)mRNA、叉头框蛋白P3(FOXP3)mRNA表达以及Th17、Treg细胞比例。Pearson/Spearman相关性分析TL1A mRNA、sPD-1 mRNA表达与Th17/Treg相关因子的关系。结果 与对照组比较,OLP组外周血单个核细胞TL1A mRNA、sPD-1 mRNA、RORγt mRNA表达水平和外周血Th17比例、Th17/Treg比值升高,外周血Treg比例、FOXP3 mRNA表达降低(P<0.05)。与非糜烂型组比较,糜烂型组外周血单个核细胞TL1A mRNA、sPD-1 mRNA、RORγt mRNA表达水平和外周血Th17比例、Th17/Treg比值升高,外周血Treg比例、FOXP3 mRNA表达降低(P<0.05)。相关性分析显示,OLP患者外周血单个核细胞TL1A mRNA、sPD-1 mRNA表达与外周血Th17比例、RORγt mRNA表达、Th17/Treg比值呈正相关,与外周血Treg比例、FOXP3 mRNA表达呈负相关(P<0.05)。结论 OLP患者外周血单个核细胞TL1A 、sPD-1 异常高表达,与Th17/Treg失衡密切相关,TL1A、sPD-1可能通过引起Th17/Treg失衡参与OLP的发生发展。

      Objective To investigate the correlation between the expression of tumor necrosis factor-like ligand 1A (TL1A) and soluble programmed death receptor 1 (sPD-1) messenger ribonucleic acid (mRNA) in peripheral blood mononuclear cells and the imbalance of helper T cell 17 (Th17)/regulatory T cell (Treg) in patients with oral lichen planus (OLP). Methods 133 patients with OLP (OLP group) who were treated in our hospital from January 2020 to December 2023 were selected, and divided into erosive group (74 cases) and non-erosive group (59 cases) according to the type of damage, another 113 healthy subjects in our hospital during the same period were selected as control group. The expression of TL1A mRNA and sPD-1 mRNA in peripheral blood mononuclear cells, the expression of retinoic acid-related orphan nuclear receptor γt (RORγt) mRNA and forkhead box protein P3 (FOXP3) mRNA in peripheral blood, and the proportion of Th17 and Treg cells were detected. The relationship between the expression of TL1A mRNA, sPD-1 mRNA and Th17/Treg related factors were analyzed by Pearson/Spearman correlation analysis. Results Compared with control group, the expression levels of TL1A mRNA, sPD-1 mRNA and RORγt mRNA in peripheral blood mononuclear cells, the proportion of Th17 and the ratio of Th17/Treg in peripheral blood were increased, and the proportion of Treg and the expression of FOXP3 mRNA in peripheral blood were decreased in OLP group (P<0.05). Compared with non-erosive group, the expression levels of TL1A mRNA, sPD-1 mRNA and RORγt mRNA in peripheral blood mononuclear cells, the proportion of Th17 and the ratio of Th17/Treg in peripheral blood were increased, the proportion of Treg and the expression of FOXP3 mRNA in peripheral blood were decreased in erosive group (P<0.05). Correlation analysis showed that, the expression of TL1A mRNA and sPD-1 mRNA in peripheral blood mononuclear cells of OLP patients were positively correlated with the proportion of Th17, the expression of RORγt mRNA and the ratio of Th17/Treg in peripheral blood, and negatively correlated with the proportion of Treg and the expression of FOXP3 mRNA in peripheral blood (P<0.05). Conclusion The abnormal high expression of TL1A and sPD-1 in peripheral blood mononuclear cells of OLP patients, which are closely related to Th17/Treg imbalance, TL1A and sPD-1 may participate in the occurrence and development of OLP by causing Th17/Treg imbalance.
