摘要 目的:观察早期胚胎停止发育孕妇血清绒毛膜促性腺激素(β-HCG)、孕酮(P)及雌二醇(E2)的表达水平,并分析早期胚胎停止发育的影响因素。方法:选择2020年4月~2022年1月期间长沙市第四医院收治的早期胚胎停止发育的患者94例作为研究组,随机选取60例来长沙市第四医院健康体检的孕妇作为对照组,对比对照组、研究组血清β-HCG、P及E2的表达水平,采用Pearson法分析β-HCG、E2、P之间的相关性。采用多因素Logistic回归分析早期胚胎停止发育的影响因素。结果:研究组的血清β-HCG、P、E2水平低于对照组(P<0.05)。Pearson相关性分析结果显示,β-HCG与E2、P呈正比,E2与P呈正比(P<0.05)。单因素分析结果显示:早期胚胎停止发育与年龄、受孕期间服用紧急避孕药、近期曾接触过有毒有害物质、家庭平均月收入、既往流产史、补充叶酸、睡眠时间<6 h/d、TORCH感染、每天接触电离辐射时间有关(P<0.05)。多因素分析结果显示:β-HCG、P、E2水平偏低、受孕期间服用紧急避孕药、年龄偏大、经常睡眠时间<6 h/d、TORCH感染、近期曾接触过有毒有害物质、每天接触电离辐射时间≥6 h是早期胚胎停止发育的危险因素,而补充叶酸、家庭平均月收入偏高是早期胚胎停止发育的保护因素(P<0.05)。结论:早期胚胎停止发育孕妇体内β-HCG、P、E2水平偏低,且受到年龄、受孕期间服用紧急避孕药、家庭平均月收入、TORCH感染、近期曾接触过有毒有害物质、既往流产史、每天接触电离辐射时间、睡眠时间等因素的影响,而补充叶酸是早期胚胎停止发育的保护因素。 |

ABSTRACT Objective: To observe the expression levels of serum chorionic gonadotropin (β-HCG), progesterone (P) and estradiol (E2) in pregnant women with early embryo stop development, and analyze the influencing factors of early embryo stop development. Methods: From April 2020 to January 2022, 94 patients with early embryo stop development who were admitted to Changsha Fourth Hospital were selected as the study group, and 60 healthy pregnant women who came to Changsha Fourth Hospital for physical examination were randomly selected as the control group. The expression levels of serum β-HCG, P and E2 in the control group and the study group were compared. The correlation between β-HCG, E2 and P was analyzed by Pearson correlation analysis. Multivariate logistic regression were used to analyze the influencing factors of early embryo stop development. Results: The levels of β-hCG, P and E2 in the study group were lower than those in the control group (P<0.05). Pearson correlation analysis showed that β-HCG was positively correlated with E2 and P, and E2 was positively correlated with P(P<0.05). Univariate analysis showed that age, use of emergency contraception during pregnancy, recent exposure to toxic and harmful substances, average monthly family income, previous abortion, folic acid supplementation, sleep duration <6 h/d, TORCH infection and daily exposure to ionizing radiation were associated with early embryo stop development (P<0.05). The results of multivariate regression showed that: low levels of β-HCG, P and E2, use of emergency contraception during pregnancy, older age, regular sleep duration <6 h/d, TORCH infection, recent exposure to toxic and harmful substances and daily exposure to ionizing radiation ≥6 h were risk factors for early embryo stop development. However, folic acid supplementation and high average monthly family income were the protective factors for early embryo stop development(P<0.05). Conclusion: The low levels of β-HCG, P and E2 in pregnant women with early embryo stop development are influenced by age, use of emergency contraception during pregnancy, average monthly family income, TORCH infection, recent exposure to toxic and harmful substances, previous history of abortion, daily exposure to ionizing radiation and sleep duration. Folic acid supplementation is a protective factor against early embryo stop development. |
