      摘要 目的:探讨缩唇腹式呼吸联合呼吸操训练对慢性阻塞性肺疾病(COPD)患者血气分析指标、肺功能和生活质量的影响。方法:选取2020年1月-2021年4月期间就诊于我院的82例COPD患者,根据随机数字表法分为对照组(缩唇腹式呼吸训练,41例)和研究组(缩唇腹式呼吸结合呼吸操训练,41例),对比两组疗效、血气分析指标[动脉血二氧化碳分压(PaCO2) 、血氧分压(PaO2)、血氧饱和度(SaO2)]、肺功能[用力肺活量(FVC) 、第1秒用力呼气容积(FEV1) 、第1秒用力呼气容积所占FVC的比例( FEV1/FVC)]、生活质量[圣乔治呼吸问卷(SGRQ)评分]、6分钟步行试验(6MWT)距离、BODE指数。结果:研究组的临床总有效率明显高于对照组(P<0.05)。研究组治疗3个月后PaO2、SaO2高于对照组,PaCO2低于对照组(P<0.05)。研究组治疗3个月后FEV1、FVC、FEV1/FVC高于对照组(P<0.05)。研究组治疗3个月后6MWT距离长于对照组,SGRQ评分、BODE指数低于对照组(P<0.05)。结论:缩唇腹式呼吸联合呼吸操训练可改善COPD患者血气分析指标,提高肺功能和运动耐力,改善生活质量,疗效显著。

      ABSTRACT Objective: To investigate the effects of lip contraction abdominal breathing combined with respiratory exercises on blood gas analysis indexes, pulmonary function and quality of life in patients with chronic obstructive pulmonary disease (COPD). Methods: 82 patients with COPD who were treated in our hospital from January 2020 to April 2021 were selected, and they were randomly divided into control group(lip contraction abdominal breathing training, 41 cases) and study group(lip contraction abdominal breathing combined with respiratory exercises, 41 cases). The efficacy, blood gas analysis indexes[arterial partial pressure of carbon dioxide (PaCO2), partial pressure of blood oxygen (PaO2), blood oxygen saturation (SaO2)], pulmonary function [forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), proportion of forced expiratory volume in the first second (FEV1/FVC)], quality of life [St George's Respiratory Questionnaire (SGRQ) score], 6-minute walking distance (6MWT), BODE index were compared between the two groups. Results: The total clinical effective rate of the study group was significantly higher than that of the control group(P<0.05). 3 months after treatment, PaO2 and SaO2 of the study group were higher than those of the control group, and PaCO2 was lower than that of the control group(P<0.05). FEV1, FVC and FEV1/FVC of the study group at 3 months after treatment were higher than those of the control group(P<0.05). 3 months after treatment, 6MWT of the study group was longer than that of the control group, SGRQ score and BODE index were lower than those of the control group (P<0.05). Conclusion: Lip contraction abdominal breathing combined with respiratory exercises can improve blood gas analysis indexes of patients with COPD, improve lung function and exercise endurance, improve the quality of life, with significant efficacy.
