      摘要 目的:探究不同分期慢性肾脏病(CKD)患者血清Klotho蛋白、成纤维细胞生长因子-23(FGF-23)、视黄醇结合蛋白4(RBP4)、肿瘤坏死因子样弱凋亡诱导因子(TWEAK)水平与肾功能的相关性。方法:选择2018年2月至2020年2月我院诊治的80例CKD患者作为CKD组,选择同期在我院进行健康体检的80名健康者作为对照组。检测并比较对照组和CKD组以及不同CKD分期患者血清Klotho蛋白、FGF-23、RBP4、TWEAK水平及肾功能指标水平,采用Pearson相关性分析对血清Klotho蛋白、FGF-23、RBP4、TWEAK水平与肾功能指标的相关性进行分析。结果:与对照组相比,CKD组血清Klotho蛋白、TWEAK水平和肾小球滤过率(GFR)明显下降,而血清FGF-23、RBP4水平、血清肌酐、24尿蛋白定量和血清尿素氮水平明显升高(P<0.05)。Ⅰ~Ⅱ期患者的血清Klotho蛋白、TWEAK水平和GFR最高,其次为Ⅲ~Ⅳ期患者,Ⅴ期患者最低(P<0.05)。而Ⅰ~Ⅱ期患者血清FGF-23、RBP4水平、血清肌酐、24尿蛋白定量和血清尿素氮水平最低,其次为Ⅲ~Ⅳ期患者,Ⅴ期患者最高(P<0.05)。Klotho蛋白和TWEAK水平与血清肌酐、24尿蛋白定量和血清尿素氮水平呈负相关,与GFR呈正相关(P<0.05)。FGF-23和RBP4水平与血清肌酐、24尿蛋白定量和血清尿素氮水平呈正相关,与GFR呈负相关(P<0.05)。结论:CKD患者中血清Klotho蛋白、TWEAK水平以及肾功能下降,FGF-23、RBP4水平明显升高,且血清Klotho蛋白、TWEAK、FGF-23、RBP4水平与肾功能及CKD分期相关。

      ABSTRACT Objective: To explore the correlation between serum Klotho protein, fibroblast growth factor-23 (FGF-23), retinol binding protein 4 (RBP4), tumor necrosis factor like weak apoptosis inducing factor (TWEAK) levels and renal function in patients with chronic kidney disease at different stages(CKD). Methods: 80 cases of patients with CKD who were diagnosed and treated in our hospital from February 2018 to February 2020 were selected as the CKD group, and 80 healthy patients who underwent physical examination in our hospital during the same period were selected as the control group. Serum levels of Klotho protein, FGF-23, RBP4, TWEAK and the renal function indexes in the control group, CKD group and patients with different CKD stages were measured and compared, the correlation between serum levels of Klotho protein, FGF-23, RBP4, TWEAK and renal function indexes were analyzed by Pearson correlation. Results: Compared with the control group, the serum levels of Klotho protein, TWEAK and glomerular filtration rate (GFR) were significantly reduced in the CKD group, while the levels of serum FGF-23, RBP4, serum creatinine, 24 urine protein and serum urea nitrogen were significantly increased(P<0.05). Serum levels of Klotho protein, TWEAK and glomerular filtration rate in the Ⅰ~Ⅱ stage patients was highest, followed by Ⅲ ~ Ⅳ period patients, Ⅴ period in patients were the lowest(P<0.05). And the Serum level of FGF-23, RBP4 24 urinary protein, serum creatinine, serum urea nitrogen in the stage Ⅰ~Ⅱ patients were the lowest, followed by Ⅲ ~ Ⅳ period patients, patients with Ⅴ period were the highest(P<0.05). The levels of serum Klotho protein and TWEAK were negatively correlated with the levels of serum creatinine, 24 urine protein quantification and serum urea nitrogen, and positively correlated with glomerular filtration rate(P<0.05). The levels of FGF-23 and RBP4 were positively correlated with the levels of serum creatinine, 24 urine protein quantification and serum urea nitrogen, and negatively correlated with glomerular filtration rate (P<0.05). Conclusion: In CKD patients, the levels of serum Klotho protein, TWEAK and renal function decreased, the levels of FGF-23 and RBP4 increased significantly, and the levels of serum Klotho protein, TWEAK, FGF-23 and RBP4 are related to the renal function and the CKD stage.
