摘要 目的:评价光动力疗法(PDT)治疗面部伴发蠕形螨病的基底细胞癌(BCC)的疗效、不良反应和复发情况,分析其对细胞免疫功能的影响。方法:124例面部伴发蠕形螨病的BCC患者接受奥硝唑抗蠕形螨治疗后分组,9例患者被排除,观察组患者56例接受PDT治疗;对照组患者59例接受肿瘤扩大切除术及局部皮瓣转移术治疗。随访2年,评估患者疗效、细胞免疫功能、不良反应和复发情况。结果:观察组总有效率与对照组比较未见显著性差异(P>0.05);两组治疗后CD4+、CD4+/CD8+水平均较治疗前升高,且观察组高于对照组,两组治疗后CD8+水平均较治疗前降低,且观察组低于对照组(均P<0.05);观察组不良反应发生率和复发率均低于对照组(P<0.05)。结论:PDT治疗对面部伴发蠕形螨病的BCC有效,在降低不良反应发生率和复发率等方面表现突出的优势,且能够改善患者的细胞免疫功能。 |

ABSTRACT Objective: To evaluate the efficacy, adverse reactions and recurrence of photodynamic therapy (PDT) in the treatment of facial basal cell carcinoma (BCC) of the face with vermiform acariasis, and to analyze the effect of them on cellular immune function. Methods: 124 BCC patients with facial Demodex were divided into two groups after ornidazole treatment, 9 patients were excluded, 56 patients in the observation group were treated with PDT, 59 patients in the control group were treated with extended tumor resection and local flap transfer. The patients were followed up for 2 years, the efficacy, cellular immune function, adverse reactions and recurrence were evaluate. Results: There was no significant difference in the total effective rate between the observation group and the control group (P>0.05). After treatment, CD4+ and CD4+/CD8+ levels in both groups were increased compared with those before treatment, and the observation group was higher than the control group. CD8+ levels in both groups were decreased compared with that before treatment, and the observation group was lower than the control group (all P<0.05). The incidence and recurrence rate of adverse reactions in observation group were lower than those in control group(P<0.05). Conclusion: PDT is effective in the treatment of BCC patients with face with vermiform acariasis. It has outstanding advantages in reducing the incidence of adverse reactions and recurrence rate, and can improve the cellular immune function of patients. |
