      摘要 目的:研究BIPAP呼吸机联合舒利迭治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的临床效果及对患者血清IL-6、IL-8及TNF-α水平的影响。方法:选取2015年8月至2016年7月我院收治的84例慢性阻塞性肺疾病合并II型呼吸衰竭患者,根据入院顺序分为观察组和对照组,42例每组。对照组使用BIPAP呼吸机完成治疗,观察组采取舒利迭配合BIPAP呼吸机治疗。比较两组患者治疗前后血气指标(pH值、二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)),肺功能指标(功能残气量(FRC)、残气量(RV)、深吸气量(IC)、肺活量(FVC)、第一秒用力呼气容积(FEV1)),生理指标(血压、呼吸频率、心率),血清IL-6、IL-8及TNF-α水平的变化及不良反应的发生情况。结果:治疗后,观察组的pH值、PaO2水平较治疗前显著升高(P<0.05),PaCO2较治疗前显著降低(P<0.05),而对照组的pH值、PaCO2、PaO2水平和治疗前相比无显著差异(P>0.05),观察组的pH值、PaO2水平显著高于对照组(P<0.05),PaCO2显著低于对照组(P<0.05);两组患者FRC、RV、IC、FVC、FEV1水平较治疗前显著升高(P<0.05),和对照组相比,观察组的FRC、RV、IC、FVC、FEV1水平较高(P<0.05);两组患者血压、呼吸频率、心率、血清IL-6、IL-8、TNF-α水平较治疗前显著降低(P<0.05),且观察组以上指标均明显低于对照组(P<0.05)。结论:应用舒利迭配合BIPAP呼吸机治疗慢性阻塞性肺疾病合并II型呼吸衰竭患者能有效改善患者肺功能,可能与其显著降低患者血清IL-6、IL-8及TNF-α水平有关。

      ABSTRACT Objective: To study the clinical efficacy of BIPAP ventilator combined with sulpiride in the treatment of chronic obstructive pulmonary disease with type Ⅱ respiratory failure and the effect on serum IL-6, IL-8 and TNF-α levels. Methods: From August 2015 to July 2016, 84 patients with chronic obstructive pulmonary disease complicated with type II respiratory failure were enrolled in our hospital. According to the order of admission, those patients were divided into the observation group and the control group, 42 groups in each group. The control group was treated with BIPAP ventilator, and the observation group was treated with sulindine combined with BIPAP ventilator. The blood gas parameters(pH value, partial pressure of carbon dioxide (PaCO2), arterial oxygen pressure (PaO2), lung function index(functional residual capacity (FRC), residual volume (RV), deep inspiratory capacity (IC), vital capacity (FVC), first-second forced expiratory volume (FEV1)), physiological indicators (blood pressure, respiratory rate, heart rate), serum IL-6, IL-8 and TNF-α levels and the incidence of adverse reactions were compared between the two groups before and after treatment. Results: After treatment, the pH value and PaO2 levels of observation group were significantly higher than those before treatment (P <0.05), PaCO2 was significantly lower than that before treatment (P <0.05), there was no significant difference in the pH value, PaCO2 and PaO2 levels in the control group before and after treatment (P> 0.05), the levels of Ph and PaO2 of observation group were significantly higher than those of the control group (P <0.05), PaCO2 was significantly lower than that of the control group (P <0.05). The levels of FRC, RV, IC, FVC and FEV1 in both groups were significantly higher than those before treatment (P <0.05). Compared with the control group, the levels of FRC, RV, IC, FVC and FEV1 in the observation group were higher (P<0.05). The levels of serum IL-6, IL-8 and TNF-α in the two groups were significantly lower than before treatment (P <0.05), and the above indexes were significantly lower in the observation group than those in the control group (P <0.05). Conclusion: Sulpiride combined with BIPAP ventilator can effectively improve lung function in the treatment of chronic obstructive pulmonary disease with patients with type II respiratory failure, which may be associated with a significant reduction in serum levels of IL-6, IL-8 and TNF-α.
