摘要 目的:探讨丹参川芎嗪注射液联合无创正压通气(NIPPV)治疗慢性阻塞性肺病COPD合并呼吸衰竭的临床疗效。方法:选择2015年1月至2016年12月在我院进行治疗的COPD合并呼吸衰竭患者70例,随机分为两组,每组35例。对照组患者采用NIPPV治疗,观察组在此基础上给予丹参川芎嗪注射液静脉滴注。比较两组患者治疗期间的动脉血气分数、肺功能指标,评价两组患者治疗前后的病情状况以及阻塞性肺病及支气管哮喘生理状况。结果:治疗后3 d以及10 d,观察组动脉血氧分压(PaO2)、一秒用力呼气容积(FEV1)、用力肺活量(FVC)以及FEV1/FVC均显著高于对照组(P<0.05),观察组动脉二氧化碳分压(PaCO2)显著低于对照组(P<0.05),而两组间动脉血氧饱和度(SaO2)、pH比较差异无统计学意义(P<0.05)。治疗后,观察组的急性生理性与慢性健康状况(APACHEⅡ)以及慢性阻塞性肺疾病和支气管哮喘生理(CAPS)评分均显著低于对照组(P<0.05)。结论:丹参川芎嗪注射液辅助NIPPV有利于COPD合并呼吸衰竭患者肺功能的恢复,改善患者血气指标和预后。 |

ABSTRACT Objective: To explore the clinical curative effect of salviae miltiorrhizae and ligustrazine hydrochloride injection com- bined with noninvasive positive pressure ventilation (NIPPV) on the patients with Chronic obstructive pulmonary disease (COPD) com- bined with respiratory failure. Methods: 70 patients with COPD and respiratory failure were enrolled in our hospital from January 2015 to December 2016 and randomly divided into two groups, the control group (n=35) was given NIPPV treatment, and the study group was given salviae miltiorrhizae and ligustrazine hydrochloride injection based on the control group. The arterial blood gas, pulmonary func- tion index were detected and analyzed in the period of treatment between two groups, the state of the disease and the physiological status of obstructive pulmonary disease and bronchial asthma were evaluated before and after treatment. Results: The arterial partial pressure of oxygen (PaO2), forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC) and FEV1/FVC of patients in study group were significantly higher than those of the control group, and arterial partial pressure of carbon dioxide (PaCO2) in study group was lower than those of control group at 3 d and 10 d post-treatment (P<0.05); no significant difference was found in the arterial oxygen saturation (SaO2), pH between two groups (P>0.05). After treatment, the acute physiology and chronic health (APACHE II) score and the COPD and asthma physiology score (CARS) of study group were significantly lower than those of the control group (P<0.05). Conclusion: Salviae miltiorrhizae and Lligustrazine hydrochloride injection combined with NIPPV improved the recovery of pulmonary function, blood gas index and prognosis of patients with COPD and respiratory failure. |
