      摘要 目的:探讨盐酸纳美芬联合醒脑静注射液对急性酒精中毒昏迷患者的疗效及血清β-内啡肽(β-EP)、超氧歧化酶(SOD)、丙二醛(MDA)水平的影响。方法:研究对象选自我院2015年2月~2016年10月收治的128例急性酒精中毒昏迷患者,采取随机数字表将其分成两组,每组64例。对照组给予盐酸纳美芬治疗,观察组联合给予醒脑静注射液治疗,对比两组临床疗效及治疗前后血清β-EP、SOD、MDA水平的变化情况。结果:观察组治疗总有效率为93.75%,与对照组(79.69%)相比明显上升(P<0.05)。观察组苏醒时间、症状消失时间均显著短于对照组(P<0.01),苏醒后恶心呕吐、头晕头痛、全身乏力、心悸胸闷、血压下降发生率分别为14.06%、12.50%、32.81%、15.63%、12.50%,与对照组相比均有明显下降(P<0.05)。两组治疗后血清β-EP、MDA水平均较治疗前显著下降,血清SOD水平显著上升(P<0.01),且观察组这3项指标改善效果较对照组更为明显(P<0.01)。两组患者治疗后血清ALT、AST、GGT水平均较治疗前显著下降(P<0.01),观察组下降更明显(P<0.01)。结论:盐酸纳美芬联合醒脑静注射液治疗急性酒精中毒昏迷患者可有效减轻机体内氧化应激反应,促进快速苏醒,疗效确切。

      ABSTRACT Objective: To study the efficacy of nalmefene hydrochloride combined with Xingnaojing Injection on acute alcoholic coma patients and the effects on serum β-endorphin (β-EP), superoxide dismutase (SOD) and malondialdehyde (MDA) levels. Methods: 128 cases of acute alcoholism coma patients admitted in our hospital from February 2015 to October 2016, who were randomly divided into two groups with the method of random number table with 64 cases in each group. The control group was treated with nalmefene hydrochloride, while the observation group was treated with Xingnaojing injection. Then the clinical efficacy of the two groups and the changes of serum β-EP, SOD and MDA levels before and after the treatment were compared. Results: The total effective rate of observation group was 93.75%, which was significantly higher compared with 79.69% of the control group (P<0.05). The recovery time and symptom disappearance time of observation group were significantly shorter than those of the control group (P<0.01). The incidence of nausea, vomiting, dizziness, headache, malaise, palpitation, chest tightness, and blood pressure were 14.06%, 12.50%, 32.81%, 15.63%, and 12.50%, respectively, which were significantly lower compared with those of the control group (P<0.05). The serum β-EP and MDA levels of both groups were significantly decreased compared with those before treatment, while the serum SOD level was significantly increased (P<0.01), the improvement effect of these 3 indicators in the observation group were more obvious (P<0.01). The serum ALT, AST and GGT levels of both groups after treatment were significantly decreased compared with those before the treatment (P<0.01), which decreased more significantly in the observation group(P<0.01). Conclusion: Nalmefene hydrochloride combined with Xingnaojing injection could effectively alleviate the oxidative stress reactions and promote the recovery in the treatment of acute alcohol coma.
