法:选取2013 年11 月到2015 年11月我院收治的胰腺占位病变患者90 例,患者入院次日行EUS-FNA,检测患者血清及活检物
中K-ras基因阳性率,比较EUS-FNA 单独与EUS-FNA联合K-ras 基因检测对胰腺癌诊断的准确率与敏感性。结果:90 例胰腺占
位病变者中,经病理证实胰腺癌56 例,EUS-FNA 单独与EUS-FNA联合K-ras 基因分别检出胰腺癌50 例、53 例,准确率分别为
89.29%、94.64%,敏感性分别为92.59%、98.15%,两组比较差异均有统计学意义(P<0.05)。胰腺癌患者活检物中K-ras 阳性率为
83.93%,明显高于血清中的41.07%(P<0.05)。结论:EUS-FNA联合K-ras基因检测可提高对胰腺癌诊断的准确率与敏感性。 |

Objective:To study the diagnostic value of endoscopic ultrasonography guided fine needle aspiration biopsy
(EUS-FNA)combined with K-ras gene detection for pancreatic cancer, so as to provide basis for clinical diagnosis and treatment.Methods:90 cases of patients with pancreatic occupying lesions were selected treated in our hospital from November 2013 to November
2015, who were underwent EUS-FNA the next day after admission, then detected the positive rates of K-ras gene in serum and biopsy,
and compared the accuracy rate and sensitivity between EUS-FNA and EUS-FNA combined with K-ras gene detection on the diagnosis
of pancreatic cancer.Results:56 patients were confirmed with pancreatic cancer by pathology detection among 90 patients with
pancreatic occupying lesions, EUS-FNA and EUS-FNA combined with K-ras gene detection found 50 cases and 53 cases respectively,
and the accuracy rate were 89.29%vs 94.64%, sensitivity were 92.59% vs 98.15%, the differences were statistically significant(P<0.05).
The positive rate of K-ras in biopsy was 83.93%, which was significantly higher than 41.07%in serum of patients with pancreatic cancer
(P<0.05).Conclusion:EUS-FNA combined K-ras gene detection could increase the accuracy rate and sensitivity for the diagnosis of
pancreatic cancer. |
