      目的:研究外源性硫化氢钠(NaHS) 对创伤失血性休克大鼠肺损伤的影响。方法:72 只SD 大鼠随机分成三组,假手术组 (Sham组,n=24),模型组(HTS 组,n=24),NaHS处理组(NaHS组,n=24)。大鼠建立创伤失血性休克模型,Sham 组完成所有手术 操作,但不放血和复苏,HTS 组完成所有手术操作放血后给予Ringer's 液复苏,NaHS组在复苏前给与NaHS 28 umol/kg (生理盐 水稀释至0.5 mL)腹腔注射,完成所有手术操作。持续监测各组平均动脉压(MAP)及心率(HR),并通过监测肺系数、肺湿/ 干重比 观察肺水肿的情况,通过Western Blot 监测SDF-1、CXCR4 蛋白水平的变化,通过ELISA测定肺组织中TNF-alpha、IL-1beta、IL-6 和 IL-10 浓度的变化。结果:① 与Sham组比较,HTS组复苏后,肺系数、肺湿/干重明显增加,SDF-1、CXCR4蛋白表达增多,肺组织 中TNF-alpha、IL-1beta、IL-6 浓度明显升高(P<0.05);而IL-10 浓度组间差异不明显(P>0.05)。② 与HTS组比较,复苏后l 小时NaHS组肺 系数、肺湿/ 干重减轻,SDF-1、CXCR4 蛋白表达增多,肺组织中TNF-alpha、IL-1beta、IL-6 浓度降低(P<0.05);而IL-10 浓度组间差异不 明显(P>0.05)。结论:外源性硫化氢可改善创伤失血性休克大鼠复苏后肺损伤,其机制可能是通过干预SDF-1/CXCR4通路及抑制 复苏后早期炎症反应。

      Objective:To investigate the effects of exogenous Sodium hydrogen sulfide (NaHS)on the lesions of lungs in rats with hemorrhagic traumatic shock.Methods:All the SD rats were randomly divided into Sham group, HTS group, and NaHS group (n=24 each group). Traumatic hemorrhagic shock model of the rats was established. The rats in Sham group were performed all the operation without bleeding and the resuscitation; and the rats in HTS group were kept bleeding after completing all the operation and then they were given Ringer' S liquid resuscitation; and the rats in NaHS group were performed all the operation and treated with injection of 28 umol/kg NaHS (saline were diluted to 0.5 per cent) i.p. before the resuscitation. The mean arterial pressure ( MAP ) and heart rate (HR) of rats were continuously monitored, and the situation of pulmonary edema was evaluated by monitoring the lung coefficient, lung wet / dry weight ratio, and the level of SDF-1 and CXCR4 protein was detected by Western Blot, and the change of TNF-alpha, IL-1beta, IL-6 and IL-10 concentration in lung tissue were measured by ELISA.Results:① Compared with Sham group, after a recovery, the pulmonary coefficient, wet / dry weight ratio were increased significantly in HTS group, and the expression of SDF-1 and CXCR4 were increased, and the concentration of TNF-琢, IL-1茁, IL-6 in lung tissues was significantly increased (P<0.05); and the difference of IL-10 concentration was not statistically significant (P>0.05); ②compared with the HTS group, after l hour's resuscitation, the pulmonary coefficient and wet / dry weight ratio in NaHS group was decreased, and the expression of SDF-1 and CXCR4 were increased, and the concentration of TNF-alpha, IL-1beta, IL-6 in lung tissue were significantly decreased (P<0.05 ), but the difference of IL-10 concentration was not statistically significant (P>0.05).Conclusion:Exogenous hydrogen sulfide can improve the lung injury after resuscitation in rats induced by traumatic hemorrhagic shock, its mechanism may be through intervention of the SDF-1/CXCR4 pathways and inhibition of the inflammatory reaction after resuscitation.
