目的：探讨miR-126在膀胱癌患者尿液中的表达与临床病理特征的关系，评估miR-126 的肿瘤标志物诊断价值。方法：收集 48 例初发膀胱尿路上皮癌患者与32 例健康对照者晨尿，提取尿液总RNA，通过实时荧光定量PCR 技术检测各样本中的 miR-126 的表达水平，并经受试者工作曲线（ROC）分析其诊断价值。结果：膀胱癌患者尿液中的miR-126 表达水平相对健康对照组明显上调（P<0.01)，其表达水平在不同病理级别之间存在显著差异（P 均<0.05），且低级别组表达水平略高于高级别组，与肿瘤大小、数目以及淋巴转移也有一定的相关性（P<0.05），而与患者的年龄、性别、TNM 分期等均无相关性（P>0.05）。通过ROC曲线分析尿液中miR-126 诊断膀胱肿瘤的曲线下面积（AUC）为0.861，当最佳切点定在7.475 时，miR-126 诊断膀胱肿瘤的敏感性和特异性分别为75.0 %、81.2 %。结论：膀胱癌患者尿液中miR-126 的表达差异能够反映病情进展程度，其表达水平对膀胱肿瘤的早期诊断及病情评估具有一定的价值。

英文摘要:

Objective:To investigate the relationship between urine miR-126 expression and the clinicopathological parameters of bladder cancer, and evaluate the diagnostic value of miR-126 as tumor marker.Methods:Urine miR-126 expression of 48 patients with bladder cancer and 32 cases of healthy controls were assayed by RT-PCR, total urine RNAs was extracted, and its association with clinical pathological features of bladder cancer was analyzed, and its diagnostic value was evaluated by the ROC curve.Results:In contrast to controls, the level of urine miR-126 of bladder cancer patients was significantly raised (P<0.01), however, it significantly negatively correlated with histologic grade (P<0.05). In addition, the expression of urine miR-126 was significantly related to tumor size, tumor number and lymph node metastasis(P<0.05), but not related to age, sex or TMNstage(P>0.05). Using ROC curve analysis the urine miR-126 in the diagnosis of bladder tumor AUC was 0.861, when set optimal cutoff point 7.475, the sensitivity and specificity of miR-126 in the diagnosis of bladder tumor was 75.0 %, 81.2% respectively.Conclusion:The expression levels of miR-126 in patients' urine of bladder cancer were significantly increased and closely related to the degree of disease progression, indicating that miR-126 may play a role in the progress of occurrence and development of bladder cancer, and has the vital significance for early diagnosis of this disease.
