      目的:探讨难治性癫痫患儿应用生酮饮食对其滤泡辅助性T 细胞及B 细胞的影响。方法:将32 例难治性癫痫患儿设为实验 组,选取正常同龄对照20 例,流式细胞术检查生酮饮食治疗前后Tfh 细胞及B 细胞比例,ELISA 检测IL-21 血清中浓度,RT-PCR 检测Blimp-1、Bcl-6、mTOR 基因表达情况。结果:生酮治疗后Tfh 细胞数量明显下降,Tfh 细胞抑制因子Blimp-1 表达降低,转录 相关因子Bcl-6 表达也降低,血清中IL-21 浓度及mRNA表达均显著减低,CD27+IgD-B细胞、CD27+IgD-BCD38bigb 与治疗后相 比显著下降。治疗后Tfh 细胞及各阶段B 细胞与mTOR 表达存在正相关关系。结论:KD 治疗通过mTOR 降低了Tfh 细胞与B细 胞比例或是抑制了Tfh 细胞与B细胞的功能,这可能是导致接受KD 治疗过程中少数患儿出现低丙种球蛋白血症原因。

      Objective:To explore the influence of ketogenic diet on the follicular helper T cells and B cells of intractable epilepsy patients.Methods:The 32 patients of intractable epilepsy were selected and 20 children at the same age as control. Before and after the ketogenic diet, the proportion of Tfh cells and B cells was detected by flow cytometry. IL-21 concentration in serum was detected by ELISA. And the gene expression of Blimp-1, Bcl-6 and mTOR were detected by RT-PCR.Results:After Ketogenic treatment, Tfh cells significantly decreased, and the expression of Blimp-1 and Bcl-6, serum concentration and mRNA expression of IL-21 were significantly reduced. The CD27+ IgD-B and CD27+ IgD-BCD38bigb cells declined significantly after treatment. Tfh cells after treatment and the B cells in each phase were positively correlated with mTOR expression.Conclusion:KD treatment reduced the proportion of Tfh cells and B cells or inhibited the function of Tfh cells and B cell function by mTOR. And it may be lead to lowgamma globulin hematic disease after KD treatment in a minority of children with intractable epilepsy.
